Description
The Community Health Worker (CHW) will be responsible for helping clients and their families to navigate and access community services, other resources, and adopt healthy behaviors. As a priority, activity will promote, maintain, and improve the health of clients and their family. Provide social support and informal counseling, advocate for individuals and Community Health needs. Community outreach, regular client contact such as home visits will be required.
The Managed CareCommunity Health Worker (CHW) collaborates with both internal clinical teams and the Community Pathways Hub to provide culturally sensitive one-on-one support and guidance to patients, families, and caretakers related to social determinant needs. The incumbent will promote patient health by assisting individuals in the adoption of healthy behaviors and serve as an advocate and liaison between clients and healthcare or social service systems
Requirements

  • Communicates to clients the purpose of program services and the potential impact of those services on their health and wellbeing.
  • Assists clients with the development of health management plans and goals.
  • Assists clients in understanding care plans and instructions, accessing needed services, and navigation of health care systems, including the securing and use of a medical home to improve access to necessary medical care and social services.
  • Coaches clients in the effective management of chronic health conditions and self-care, while motivating them to be active, engaged participants in their overall health.
  • Facilitates individual and/or group education and support programs, to improve client skills for self-management of chronic conditions and to promote healthy lifestyle behavior changes.
  • Assists with data collection and reporting in accordance with current organizational guidelines.
  • Is able to work both independently and collaboratively.
  • Complies with all procedures and policies that are mandated by the community pathways hub including home visitation and documentation timeliness.
  • Works effectively with individuals form diverse backgrounds in reducing cultural and socio-economic barriers between clients and available institutional resources.
  • Increase access to primary care through culturally competent outreach and enrollment strategies.
  • Educate clients on disease prevention and assist clients in self-management of chronic illnesses and medication adherence.
  • Build community capacity by addressing the social determinants of health and identify individual and community needs.
  • Conducts regular and timely home visits to clients active on the incumbent's caseload.
  • Completes visit documentation and checklists in a timely manner.
  • Collaborates with clinical personnel assigned to the client to ensure comprehensive, coordinated, and culturally competent care delivery.
  • Acts as client advocate and liaison between the client and community organizations and healthcare delivery systems.
  • Complete and maintain accurate written and electronic database records in accordance with reporting standards.
  • Plan and organize own work to meet priorities and deadlines (daily, weekly, monthly, quarterly, yearly)
  • Confer with supervisors regarding progress, concerns, and recommendations.
  • Ensure confidentiality practices are always maintained.
  • Participate in staff training and meetings.
  • Continuously update and improve professional competence including current or future required standards.
  • Comply with state and federal regulations that pertain to the position (e.g., OSHA: Occupational Safety Hazard Administration, ORC; Ohio Revised Code).
  • Be a positive role model for all health center staff.
  • Other duties as assigned.

Salary Description
