Looking for a full-time, bilingual Community Health Worker who can brighten the room with their energy and bring extensive support to our patients.
- Must have at least 3-5 years of experience
- Must speak SPANISH
- Work setting: In-person.

This role includes delivering health information, advocating for members, and to act as a front-line public health worker for our Enhanced Care Management (ECM) patients in the community. The primary focus is to support members in navigating the healthcare system, overcoming barriers to care, and fostering collaboration with their medical/behavioral health teams through in-person or virtual engagement.
- Assists members and their family to understand the healthcare system and find helpful resources with special attention to hard-to-reach members.
- Assists in education about chronic medical and behavioral health conditions to improve health literacy.
- Provides informal education, counseling, support, and follow-ups on connecting patients with the appropriate health services, including accompaniment to doctor’s appointment and follow up appointments.
- Assist the RN/LVN Care Manager, Behavioral Health Care Manager, and Health Program Manager in panel management by performing delegated activities as assigned. Assist with scheduling patients with other ECM Care Team Patients and care team as appropriate.
- Collaborates on patient care issues with other ECM Team Members, participating in weekly systematic case reviews and ad hoc case reviews, and consults with RN/LVN Care Manager, and/or the Behavioral Health Care Manager before taking any action that is clinical in nature.
- Engage those eligible but not yet enrolled, as well as those currently enrolled in ECM to follow-up after discharge.
- Distribute health promotion materials.
- Advocates for the patient’s concerns about their healthcare services and target the specific need for better services.
- Certification: Completion of an approved Community Health Worker Program within six (6) months of hire.
- License: Valid California Driver’s license and valid automobile insurance. Must qualify and maintain a driving record per Mission Pediatrics standards
- Knowledge and understanding of diverse populations to interact and communicate effectively with different cultural backgrounds.
- Communicate effectively with patients and families, facilitate dialogues, listen to community needs, and present community needs to healthcare providers.
- Think critically to identify difficulties and problem solving.
- Highly organized, time management, maintain patient’s records, planning health programs to educate the patient and parents, outreach efforts and coordinate health care services with providers.
- Engage members in various settings, including on the phone, at the member’s home, in hospitals, and other settings.
- Develop relationships with community members and leaders, including in the faith-based community.
- Excellent interpersonal, teamwork relationship, and conflict resolution skills
- Two (2) years of experience as a Community Health Worker, Promotora, or Health Navigator, or two (2) years in community outreach, customer service, or within a medical office, behavioral health, or substance use disorder program.
