Description

SUMMARY:

The Community Health Manager (CHM) has a variety of responsibilities, primarily to manage the ongoing operations of the Community Health Worker and Enrollment programs. These responsibilities include identifying new sites for health center promotion; scheduling events; managing direct staff; operating the mobile program, and interfacing with potential partners. The CHM plans and schedules all outreach and enrollment activities and ensures daily coverage at health fairs, swap meets, health events, and community events. The CHM assists patients, their families, and community members to navigate and access community services and adopt healthy behaviors and works cohesively with internal departments to achieve integral and coordinated care. These services include but are not limited to patient education and linkages to community resources with the aim of educating and empowering patients and their families to control symptoms and thrive. In addition, will work to identify, recruit, train and sustain the Promotora model.

ESSENTIAL JOB DUTIES AND RESPONSIBILITIES:

  1. Supports and implements the organization’s vision, mission and values.
  2. Determines priorities and method of completing daily workload to ensure that all responsibilities are carried out in a timely manner.
  3. Performs all job functions in a professional and courteous manner. This includes answering all general phone calls timely and providing excellent customer service to internal and external customers.
  4. Fosters and promotes a culture of service excellence and accountability.
  5. Acts as a liaison or bridge between patients/families and health providers/professionals.
  6. Establishes trusting relationships with patients and their families and provides general support and navigation within the health centers and community.
  7. Establishes and develops relationships with other organizations of the community.
  8. Maintains expertise in eligibility, enrollment and program specifics for all county and state programs such as, Covered CA and Medi-Cal and all programs accessible to our community.
  9. Assists eligible families with completing and submitting the appropriate health insurance application/s, including assistance with gathering necessary documents and resolving problems.
  10. Identifies, recruits, and retains health advocates and Community Health workers/promotoras.
  11. Maintains clear and concise files for grant deliverables.
  12. Implements outreach and education strategies and conducts health outreach and education events.
  13. Maintains monthly calendar of health-related events for outreach.
  14. Develops and implements specific campaigns aimed at reducing and preventing social and health related problems in the community.
  15. Provides patient education to patients/families in both individual and group settings.
  16. Provides referrals and connects patients/families to community resources.
  17. Acts as an advocate in the community on behalf of patients/families to assist in resolving barriers that may interfere with health promotion, disease prevention, and general well-being.
  18. Serves as a liaison with the Mexican Consulate and other community partners.
  19. Organizes and conducts neighborhood meetings, forums, and other community activities.
  20. Assists in the implementation and training of skills training sessions with community residents on topics such as environmental approaches to disease prevention, group leadership, effective communication and listening skills, project planning and evaluation and community organizing.
  21. Provides appropriate referrals to prevention related services to community residents.
  22. Complies with organizational policies and procedures.
  23. Performs all other duties as assigned including participation in special projects and research, and student supervision.
  24. Must be willing and able to work at all locations as needed to meet patient care needs.
  25. Must be willing and able to work all business hours including evenings and weekends.

Qualifications

QUALIFICATIONS:

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. EDUCATION/EXPERIENCE:

  • Strong verbal and written communication skills.
  • 2 years of experience managing CHW teams (or similar role) in a community-based or health plan setting preferred.
  • Bilingual English and Spanish required.
  • Good understanding of healthcare policy and regulations, as well as current trends and best practices in CHW programs.
  • Ability to communicate with a variety of audiences such as government program monitors, community agencies' staff members, and members of the target community.
  • Excellent networking skills for the purpose of arranging and coordinating presentations.
  • Ability to give presentations in both English and Spanish.
  • Strong leadership skills and the ability to motivate and lead a team.
  • Valid California driver license, good driving record and proof of car insurance.
  • Available to work evenings and weekends.

LANGUAGE SKILLS:

Ability to read and interpret documents, such as policies and procedures, routine mail, simple contracts, and procedure manuals. Ability to compose routine reports and correspondence. Ability to speak effectively with employees, visitors and management.

MATHEMATICAL SKILLS:

Ability to calculate figures and amounts such as discounts, interest, proportions, percentages, and apply concepts of basic math.

REASONING ABILITY:

Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations. Ability to apply common sense to daily situations that arise.

  12. Must have own automobile, Valid California Driver License, and active automobile insurance.
  13. Must be willing and able to work at all locations as needed to meet patient care needs.
  14. Must be willing and able to work all business hours including evenings and weekends.

The noise level in the work environment is often low. However, there are many times when there is a high ambient background noise of phones and multiple conversations. Must be able to screen out the background noise to concentrate on the work at hand.
