MAJOR JOB RESPONSIBILITIES (50%)

- Provide education to the community related to end of life issues and potential resources.
- Contribute to the mission of the DHCH and health system by participating in academic activities such as committees, conferences, publications, teaching and research.
- Maintain records and statistics in accordance with department, DHCH health system policies.
- Participate in DHCH continuous quality improvement efforts.
- Maintains confidentiality of employee and patient information.
- Exhibits an attitude, which promotes harmony and goodwill in the workplace.
- Displays a clean, neat, professional appearance.
- Participates in Performance Improvement Program.
- Promotes quality, comprehensive services through a team approach.
- Perform other related duties as assigned.
SOCIAL WORK LEAD RESPONSIBILITIES (50%)
Program Oversight to include day to day resource allocation, coordination of services, address program need and quality of care, staff counseling, supervision of front line staff including field supervisory visits, leading commit to zero Tier 1 Huddle, lead group work, staff scheduling.- Strategic Management to include assisting Assistant Director to facilitate change and evaluate outcomes;
- Financial Management with evaluation of productivity, adherence to Availability for Work Policy, timely completion of documentation.
- Human Resource Management to include evaluation of staffing patterns and needs, adherence to labor laws, staff recruitment and retention, staff development from hire through precepting; staff education and needs assessments; promote positive team dynamics; back-up approval for staff in API; providing coaching, counseling and progressive counseling to address staff performance issues.
- Performance Improvement to include data collection and evaluation, patient/customer engagement, assess customer satisfaction; manage incident reporting; monitor and promote workplace safety; respond to patient complaints, work with manager to develop performance improvement for staff.
