Job Title: Clinical Pharmacist

Mosaic Pharmacy Service is a “closed door” pharmacy that provides comprehensive pharmacy care to medically complex and vulnerable seniors, across the country. We provide a patient-focused, pharmacist-driven care model in collaboration with health systems and health plans. Our goal is to help our patients feel better about the medications they take every day by simplifying medication regimens, improving adherence, and driving positive outcomes. Job Overview

The Clinical Pharmacist will engage with Mosaic members and providers to ensure drug regimens are safe, simple, effective, and affordable. Opportunities for clinical intervention include initial and transitions of care medication reconciliation, monthly midcycle reviews, educating members, and other Medication Therapy Management activities. The Clinical Pharmacist will empower and engage members to positively impact medication adherence and provider prescribing practices. Clinical interventions are guided by the company’s Therapeutic Decision Support technology.


Primary Responsibilities

  • Provide comprehensive medication reviews and medication therapy management (MTM) services, communicating with both members and providers.
  • Apply evidence-based medicine and national guidelines when developing medication plans and recommendations.
  • Analyze and review medication profiles for potential and actual drug-related problems, including drug interactions, unnecessary therapy, etc.
  • Promote clinically appropriate prescribing practices based on evidence-based medicine and national guidelines through various modalities (member drug profile reviews, medication protocols/criteria, and case-by-case intervention).
  • Follow up with patients and providers to check on progress toward meeting drug treatment goals.
  • Serve as a drug information resource to members and pharmacy staff.
  • Keep accurate documentation of medication plans and patient progress.

Secondary Responsibilities

  • Research, develop, and implements drug utilization and disease state management strategies and intervention techniques to deliver high-quality cost-effective healthcare.
  • Assist manager with developing/updating policies and procedures and implementing changes to comply with state and federal regulations.
  • Complete pharmacist verification of prescriptions, as needed.
  • Perform other duties as assigned.

Skills/Abilities

  • Clinical pharmacotherapy expertise for common chronic conditions and medications.
  • Ability to apply pharmacotherapy principles in direct patient care.
  • Strong knowledge of drug information resources.
  • Strong listening and communication skills.
  • Ability to effectively collaborate with others and embrace teamwork.
  • Attention to detail with proven documentation skills.
  • Proficiency with Microsoft Office; especially Excel, PowerPoint, and Word.

Work Schedule

This is a full-time position with 8.5-hour shifts within the business hours of 8am-7pm, Monday-Friday. The training period for this position is 6 weeks on-site in Sterling, VA. Upon completion of training, and with approval from the supervisor, the position will become hybrid with 2 days remote and 3 days onsite (although, this may change depending on the needs of the business).


Basic Qualifications

  • Doctor of Pharmacy or Bachelor of Pharmacy from an accredited college of pharmacy.
  • License to practice pharmacy in the state of Virginia.
  • 3+ years of professional clinical pharmacy experience,
    • OR completion of residency/fellowship and 1+ year of professional clinical pharmacy experience.

Preferred Qualifications

  • Board Certification in Ambulatory Care, Geriatric Medicine, or Pharmacotherapy

  • Occasional overtime required.
