Description

The Clinical Pharmacist - Ambulatory reflects the mission, vision, and values of NMHC, adheres to the organizations Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.

Responsibilities:

  • Meets with patients individually to provide medication support and monitoring either by 1) managing directly or 2) in partnership with physicians, advanced practice nurses, physician assistants, dieticians, medical assistants, care coordinators, and other members of the Ambulatory Care Clinic team.
  • Utilizes and communicates the appropriate prescribing of medications and vaccines consistent with national clinical guidelines, evidence-based medicine, and best practices.
  • Reviews medication profile to ensure safety and proper use of medications.
  • Alert to actual or potential medication affordability/adherence issues, interactions, therapeutic duplications, medication allergies, and potential medication errors.
  • Triages and solves medication related problems demonstrating initiative and sound judgment.
  • Evaluates and documents that the medication(s) selected are appropriate for achieving the desired therapeutic outcome.
  • Selects appropriate drug formulations to achieve maximal absorption based on bariatric procedure.
  • Communicates potential problems and likely solutions to the prescriber.
  • Adjusts therapy as indicated by patient condition and laboratory results.
  • Educates patients and their families about medications.
  • Communicates professional knowledge effectively in both oral and written forms to patients and other health professionals.
  • Maintains a comprehensive EPIC patient profile. The profile will include pertinent information which may influence medication therapy decisions.
  • Maintains a current medication list in EPIC by performing medication reconciliation at each patient visit.
  • Documents all patient activity in the EPIC system.
  • Collects data, maintains logs, records and other documentation as required by the center.
  • Bills for clinical services provided, where appropriate.
  • Develops communication materials to support clinical initiatives, as needed.
  • Presents clinical information to patient groups, as needed.
  • Participates in Medical Resident, Pharmacy Resident, and Pharmacy Student Training Responds to emergency situations to assist with medication management (e.g. response to cardiac arrests).
  • Participates in quality improvement and medication use review activities of the center.
  • Organizes, prioritizes, and plans workload within given time constraints.
  • Works cooperatively with other members of the team to assure established center objectives are accomplished.
  • Ensures compliance with hazardous waste collection and disposal procedures.
  • Maintains proficiency and expertise in area/s of practice through continuous professional development.
  • Develops and maintains relationships with all customers.
  • Performs other duties as assigned.

Qualifications

Required:

  • Current license to practice pharmacy as a Registered Pharmacist in the State of Illinois.
  • Doctor of Pharmacy Degree.
  • One or more years working as a clinical pharmacist in an ambulatory setting with a focus on diabetes.
  • Experience using EPIC Ambulatory electronic medical record.

Preferred:

  • Completion of a PGY-1 or PGY-2 pharmacy residency program or equivalent experience.
  • Board Certified Pharmacotherapy Specialist (BCPS) or Board Certified Ambulatory Care Pharmacist (BCACP).
  • Certified Diabetic Educator (CDE).
  • 2 or more years of previous experience and demonstrate clinical competence in the management of key medical conditions encountered in the ambulatory environment including asthma/COPD, diabetes, heart failure, hypertension, and mental health.
