Job description

Position Summary:
Clinical Hospital Pharmacist by using established policies and procedures is responsible for: drug therapy of assigned patients; provision of clinical drug information to meet the needs of the physician, nurse, and patient, education of pharmacy students, residents, and interns; and, distribution of medications.
Duties & Responsibilities:
MS: Evaluates medication therapy for appropriateness, taking into account: a. Relevant patient conditions including patients age, allergies, renal status, other disease states etc. b. Safety and potential efficacy of the prescribed medication identity, strength, dose, frequency, duration of therapy, and directions of use; c. Formulary status and availability of prescribed medication; d. Potential for concerns related to drug-drug or drug-food interactions; e. potential alternative therapy (less costly or therapeutically superior). Provides therapeutic drug level monitoring when appropriate. Effectively communicates with nurse/prescriber when errors are detected or to optimize therapy. When needed, consults with other practitioners (other Clinical Hospital Pharmacists, Pharmacy Management, Attending Physician, etc.) as appropriate.ES: Identifies trends during routine provision of pharmaceutical care, collects data supporting concern, and formulates practice recommendations that improve outcomes or reduce cost. Presents data to the appropriate committee as required. Exceptional feedback from medical staff as it relates to the effect of the pharmacists on patient outcomes. Performs formal, written consults. Documents all appropriate clinical consult information in the medical record using the standard hospital guidelines. MS: Demonstrates accuracy when dispensing or preparing or mixing or labeling product or when filling automated dispensing machines or with computer data entry.ES: Identifies ways to decrease the possibility of medication dispensing errors. MS: Completes tasks with appropriate speed yet not sacrificing accuracy.ES: Is so efficient in completing required tasks that is able to accomplish additional ones upon request. MS: Demonstrates reliability. Performs tasks in a conscientious, thorough and complete manner. Demonstrates a needs no reminder attitude after directions have been explained and carries out work assignments promptly. ES: Identifies opportunities to improve on the quality of the outcome of the task or work assignment. MS: Demonstrates flexibility. Is willing to help on any work that needs to be done. Adapts to modifications of work assignments and, when requested, is willing to adjust personal schedule to complete work for the benefit of patient care or departmental needs. When requested, completes work left from other shifts and assists others.ES: Adapts to modifications of work assignments with enthusiasm and takes initiative to offer adjustments in personal schedule or to assist others to complete work for the benefit of patient care or departmental needs. MS: Demonstrates effective time-management skills. Coordinates tasks carefully and avoids non-essential interruptions. Completes priorities first. Meets deadlines. ES: Regularly completes tasks prior to deadlines. MS: Demonstrates good communication skills. Communicates inability to complete tasks to supervisors in a timely manner. Appropriately communicates tasks/issues that require follow-up to co-workers or supervisor. Seeks timely clarification or guidance as appropriate.ES: Takes the initiative on improving communication between staff without specific request from the supervisor or manager. MS: Demonstrates problem-solving skills. Performs routine duties without direct supervision. Brings concerns to appropriate superior and offers solutions. Understands the intent of policies and makes reasonable independent decisions including exceptions to policy when such policy would cause detriment to the patient. Demonstrates good judgment in handling situations not covered by policies and procedures.ES: Assists supervisors by suggesting changes in procedures, programs or systems to improve service, patient outcomes, customer satisfaction, reduce cost, enhance revenue, secure JMH assets, etc. MS: Develops the skills of others. May be responsible, as assigned, for: orientation of new personnel, active participation in staff development programs, education and training of pharmacy students, interns, residents, nursing, medical and /or other healthcare personnel. Helps to improve new and junior staff performance through coaching sessions. Meets area-specific quantitative requirements of these tasks. Satisfactory evaluations of the quality of these tasks are obtained consistently.ES: Volunteers for additional teaching assignments and evaluations are consistently exceptional. Consistently evaluates and offers suggestions for continued enhancement of training programs. Identifies the need for and initiates coaching sessions with junior pharmacists and technicians; maintains written documentation of these sessions with recommendations to area supervisor. Makes one presentation per year at local or national meetings or has published in an internal or external publication at least every other year. Makes one journal club and/or one in-service presentation per year. MS: Authorship in two minor publications annually, one may be internal to JMH (i.e., poster, platform, letters to the editor or case reports, newsletter article, etc.) or one major publication annually (review article, descriptive articles of practice improvement etc.). Must be accepted (or submitted) for publication.ES: Paper is accepted by prestigious international/national peer-reviewed journal or participation in the publication of original clinical research or has leadership role in the creation of a JMH publication (editor, co-editor, columnist, advisory board) or submits more than two minor publications annually. MS: Active participation in multidisciplinary administrative or clinical committee.ES: Serves as secretary, chairperson, or provides formal administrative support to a committee or actively participates in multiple committees each year. MS: Exhibits proficiency in the use of area-specific technology (i.e., automated dispensing machines, inpatient and outpatient computer systems, IV hoods, prepackaging machine etc.) and techniques (i.e., compounding, calculations, aseptic technique etc.)ES: Consistently evaluates and offers suggestions for continued enhancement of area-specific technology. Solves technical problems and turns difficult technical situations into positive action. MS: Identifies potential areas for DUE or Target Drug Program (TDP) activities and develops criteria for these activities. Presents activities to the appropriate committee as required. DUE or TDP is relevant and practical. Results lead to changes in practice/standards. Takes initiative and is directly responsible for the development and implementation of a new service or program involving productivity, QA or PI.ES: Actively participates in multiple DUE/TDP/PI activities or the scope of one activity is such that its impact has major effect on clinical outcomes or cost containment (i.e., chosen for presentation at EPIC or JCAHO etc.). MS: Complies with required documentation activities such as: Adverse Drug Reactions, Clinical Pharmacy Interventions in HealthProLink, Incident Reports, Medication Errors, etc. MS: Supervises supportive personnel assuring efficient operation of the area. Evaluates and adjusts their workflow to optimize operational activities. Assures their compliance with routine, daily tasks. Assists in their performance evaluation as requested.ES: Consistently evaluates and adjusts their workflow to optimize operational activities. Performs other related duties as required. Demonstrates behaviors of service excellence and CARE values (Compassion, Accountability, Respect and Expertise).
Qualifications
Experience:
Generally requires 5 to 7 years of related experience.
Education:
Pharm.D. degree and residency training or equivalent proven experience is required.
License Certification:
Possession of or eligible for Florida pharmacist license is required.
Knowledge Skill Abilities:
Ability to analyze, organize and prioritize work accurately while meeting multiple deadlines. Ability to communicate effectively in both oral and written form. Ability to handle difficult and stressful situations with professional composure. Ability to understand and follow instructions. Ability to exercise sound and independent judgment Knowledge and skill in use of job appropriate technology and software applications. Ability to demonstrate literacy in using computerized information systems.
Physical Demands:
Job function requires extended standing or walking. Must be able to carry objects weighing 21 to 50 pounds. Able to lift or move objects weighing 51 to 100 pounds with the use of appropriate devices. Jobs in this group require frequent visual acuity to perform activities related to extended use of computers. Work Environment:
Jobs in this group are required to function in a fast paced environment with occasional high pressure or emergent and stressful situations. Frequent interaction with a diverse population including team members, providers, patients, insurance companies and other members of the public. Function is subject to inside environmental conditions, with occasional outdoor exposures. Exposure to various environments such as: communicable diseases, toxic substances, medicinal preparations and other conditions common to a hospital and medical office environment. May also be exposed to needle sticks, airborne infections, medical gases, X-Ray, chemical exposures and other potential hazards. Must wear Personal Protective Equipment (PPE) when exposed to infectious/Clinical Hospital environment.
