POSITION SUMMARY:

Ensures availability and proper handling of formulary medications; oversees accurate dispensing and compounding (when necessary) of daily medicines; responsible for the safe, accurate and rational distribution of pharmaceutical agents to patients of all ages, employees and designated areas of the hospital, as well as the supervision of technical personnel in accordance with all state and federal laws, JCAHO standards, and hospital/departmental policies and procedures. Maintains Pharmacy Module and serves as a liaison between Pharmacy and other departments for eMAR.

ESSENTIAL CORE FUNCTIONS:

  • Demonstrates knowledge of human growth and development, assessment, range of treatment, and care of patients appropriate to population served.
  • Provides patient care activities to ensure safe and effective drug therapy through the provision of clinical services.
  • Coordinates and oversees the delivery of pharmaceutical services.

PERFORMANCE EXPECTATIONS:

· Provide Pharmacokinetic dosing when consulted

· Perform IV to PO conversions

· Perform renal dosing

· Appropriate aseptic technique applied and annual media validation passed

· Provide Anticoagulation dosing when consulted

  • Supports and assists in meeting departmental goals
  • Performs assigned department based monitoring duties in a timely manner.

  • Gives a thorough, concise report at change of shift, within allotted time. Communicates incomplete assignments, and potential patient concerns to monitor in order to ensure continuity of medication therapy.

  • Accountable for maintaining and improving own job knowledge, skills and competence.
  • Completes annual education requirements and competencies/skills assessments as needed. TB test as scheduled.
  • Education profile is completed and updated on an on-going basis on the individual attendance program form.
  • Attends all staff meetings on scheduled work days and if scheduled off, reads and signs-off the minutes of those meetings.
  • Attends all mandatory in-services as scheduled.
  • Participates in department or hospital committees, task forces, and/or provide departmental education.
  • Assists with the orientation of new employee(s) and/or orients current employee(s) to improve in a specific area.
  • Demonstrates knowledge and use of department QI by identifying problems and offering solutions.
  • Identifies QI activities by articulating department and hospital wide quality improvement successes.
  • Reviews all orders to assure appropriateness and intervenes and makes recommendations as needed. Assures departmental procedures are followed when non-formulary drugs, target drugs, restricted drugs, and investigational drugs are ordered.
  • Assures that medications are properly prepared and dispensed to the unit in a timely manner. Checks all doses prepared by technicians
  • Demonstrates necessary skills and knowledge to prepare medications. Pharmacist does not have an unrealistic amount of medication errors when compared to peers.
  • Assures that all I.V. orders are processed and prepared in a timely and accurate manner.
  • Uses proper technique when working in I.V. hoods according to procedure.
  • Reviews medication administration records to identify problems with administration or compliance with prescribed therapy.
  • Investigates inconsistencies in drug therapy including recognition and documentation of possible adverse drug reactions.
  • Participates and documents information as requested for Medication Use Evaluations or Drug Studies.
  • Issues controlled substances to patient care areas and maintains records as required by law.
  • Answers the telephone, identifying self and department.
  • Performs essential duties of the pharmacy director in his/her absence.
  • Maintains safe, appropriate, cost effective medication therapies for patients according to established policies, procedures, and protocols.
  • Reviews and interprets all physician orders received, using the patient profile. Screens all medication for clinical appropriateness and intervenes when appropriate:
  • Monitors drug therapy regiments for contraindications, drug-drug interactions, drug-food interactions, allergies, and appropriateness of drug and dose.
  • Renal dosing on specified drugs-checks initial dose.
  • Duplicate orders discontinued.
  • Changes IV orders to PO when appropriate.
  • Monitors for incompatibilities, concentration and rate.
  • Pharmacokinetic monitoring for appropriateness.
  • Anticoagulation monitoring for dosing and lab appropriateness
  • Evaluates subjective and objective responses to drug therapy to determine effectiveness. Checks drug levels when appropriate.
  • Documents clinical interventions.
  • Reviews/interprets culture and susceptibility data for antibiotic appropriateness and recommends changes as needed.
  • Sustains the formulary by minimizing non-formulary procurements, utilizing therapeutic substitution protocols, and prompting rational drug therapy selections
  • Provides clinical consultation and clarification to practitioners including accurate and timely drug information. Suggests appropriate, cost effective therapeutic alternatives to medical staff.
  • Participates in department or hospital committees, task forces, and/or provides departmental education
  • Assists with orientation of new employees and /or orients current employees to improve in a specific area
  • Assists with the development and implementation of an effective medication quality plan. May include determining usage patterns for drugs and setting drug use criteria.
  • Investigates inconsistencies in drug therapy including recognition and documentation of possible adverse drug reactions.
  • Participates and documents information as requested for Medication Use Evaluations or Drug Studies.
  • Orients, trains, and mentors pharmacists to carry out their job responsibilities as it relates to clinical pharmacy.
  • Coordinates and serves as an information source for medical and hospital staff regarding the effects and use of medication.
  • Promotes and recommends appropriate, cost-effective alternatives to drug therapy. Performs cost benefit, cost effectiveness analysis on alternative drug therapies and drug delivery systems making specific recommendations through hospital committees.
  • Promotes the utility of clinical pharmacy services toward patient outcomes and cost savings through the management of the clinical program.
  • Designs, implements, monitor and evaluate drug cost management programs in collaboration with the medical staff, administration, and pharmacists.
  • Acts as a pharmacy preceptor.
  • Performs other duties as deemed necessary by the Director in accordance with established policies and accrediting agencies.
  • Interfaces with physicians and nurses to develop quality, cost effective therapeutics through clinical pathways.
  • Works with Education Specialist to design and develop base education/training materials in pharmacy to support staff training and continue education
  • Actively supports and promotes system standardization, utilization and best practices
  • Participates with peers in the other Divisions identifying and evaluating best practices, packaging and promoting best practice efforts
  • Promotes system security and patient confidentiality and helps ensure compliance
  • Participates in special projects as needed and performs other duties as assigned.
  • Provides monitoring and intervention in therapeutics to provide cost effective care with optimum outcomes

· Reviews all orders to assure appropriateness and intervenes and makes recommendations as needed.

· Screens all medication for clinical appropriateness and intervenes when appropriate; allergies-appropriately documented in computer

· Exhibits expertise in dosing geriatric, pediatric or renally comprised patients, as well as the common population.

· Makes timely and appropriate recommendations of antibiotics based on C&S information, recommendations to adjust dose if renal function comprised, and recommendations to switch IV medications to oral when criteria are met.

EDUCATION REQUIRED AND/OR PREFERRED: Graduate of an accredited school of pharmacy

 

LICENSURE/CERTIFICATION: Florida Pharmacy License

EXPERIENCE REQUIRED OR PREFERRED: 1-2 years ofprevious hospital experience preferred

SKILLS, KNOWLEDGE, AND ABILITIES: Ability to work in a fast pace and to prioritize multiple assignments/projects and respond to numerous requests; ability to resolve conflicts among staff and to work collaboratively with department director, physicians and other members of the healthcare team; ability to ensure operational efficiency; ability to problem solve and make decisions; ability to exercise self-control and tolerate stress when dealing with multiple requests and/or conflicting demands from multiple customers. Knowledge of safe medication practices is required.

CUSTOMERS SERVED: All employees, staff of Gulf Coast Regional Medical Center in both clinical and non-clinical departments, including patients/families, physicians, volunteers, agency, contract, students and the community.
