Acute Long-Term Care Hospital
An exciting opportunity in a professional hospital setting to assist patients transition from short-term to post-acute care venues, including inpatient rehabilitation, skilled nursing, and home care. Grace Hospital is a not-for-profit registered Acute Long-Term Care Hospital, the first of its kind to be established in Cuyahoga County. We are Medicare and Medicaid certified and fully accredited by the Joint Commission. Grace Hospital utilizes an interdisciplinary care team model to produce optimal clinical outcomes. We seek care-givers who are compassionate, competent, and results-oriented to join our team.
Job Summary:
Coordinates clinical nutrition services to all patients including assessment care planning and teaching. Is Responsible for the quality improvement function of the Nutrition Services Department.
Qualifications:
Education: Bachelor's Degree from an accredited institution; Master's Degree in Dietetics is preferred. Experience: Three years experience in acute/critical care preferred.
Other: Registered with the Commission on Dietetic Registration (RD); Licensed by the State of Ohio (LD).

Qualifications
Responsibilities:
- Consistently exhibits ethical behavior in the performance of job duties.

- Follows Hospital infection control protocol and reporting guidelines.
- Is knowledgeable of and adheres to Hospital safety and emergency response protocols.
- Maintains competence and licensure/certification, as appropriate to position requirements.
- Participates in and successfully completes education required by the Hospital.
- Acquires, stores, and processes information according to the requirements of position and with respect to confidentiality and compliance considerations.
- Respects the rights of patients to be informed of and to make decisions regarding their care, treatment, and services.
- Adheres to Hospital protocol in the assessment and reassessment of patients, including special circumstance patients such as those who may be victims of abuse or those who are being treated for substance abuse and/or behavioral disorders.
- Participates in the development of patient care plans, individualized to and appropriate for the patients' needs, per hospital protocol.
- Performs patient care as detailed in the individual's care plan.
- Participates in age-appropriate patient education and training as per Hospital protocol.
- Adheres to hospital protocol for the appropriate discharge of patients.
