Position Description: The role of the Clinical Care Manager is to provide high-quality, trauma-informed care management services for adults and families that speak Spanish and English. The Clinical Care Manager will work with community members who are in need of Centro Sanar mental health services and have a variety of care management needs. Community members are often experiencing mental health symptoms due to unaddressed complex trauma histories and/or due to living within oppressive systems that have failed to meet their basic needs. This position will accompany, connect, and advocate for client needs and ensure connection to resources. Our care management model seeks to come alongside community members and help them identify their strengths and abilities while also teaching them how to access the necessary services. This position is a hybrid position that also includes outreach responsibilities. In order to connect with potential clients, the expectation is that the Clinical Care Manager be able to present about Cento Sanar services in various community settings including but not limited to schools, churches, other community organizations, and within other Centro Sanar services. The range of topics could include: our services, our service model, mental health related topics and topics that will make community members curious about engaging with Centro Sanar. The Clinical Care Manager will be part of a larger team composed of: student interns, an intake coordinator, individual and family support clinicians, clinical supervisors and a clinical director. Centro Sanar’s mental health services are specifically designed to serve individuals and families in Brighton Park, Gage Park, Chicago Lawn, and West Lawn.

The applicant must have experience offering care management to individuals in the Latinx community and be bilingual and bicultural in Spanish and English. Responsibilities will include but not be limited to conducting intake calls and assessments; maintain clinical case management caseload, conduct outreach, research resources, offer in-person accompaniment to client and data management. Because the clinical case manager will be working with adult and family schedules, a minimum of 1-2 evening shifts (12 pm to 8 pm) will be required and/or some saturday availability.

Essential Functions (list in order of importance including measures)

Provides direct individual case management to a caseload of weekly clients. Case management services will include but not be limited to accompanying, researching and advocating for resources according to client needs, conduct “intentional connections or handoffs” and assistance in completing various enrollment or service related paperwork to meet client needs. 40%

Provides off-site educational workshops and services, direct in-home or community outreach 25%

Actively contributes to and participates in supervision, staff meetings, in-services, and mental health coalition meetings. 10%

Documents all interactions and services provided in the client's case file and manages files according to regulatory requirements. 10%

Participates in program planning and evaluation activities. Is also accountable for monitoring, documenting, and tracking program and client data and assuring compliance with grant-related or non-profit related documentation requests made by supervisor. 10%

Attends training and workshops assigned to provide culturally appropriated and high quality services to the population served. 5%

Performs other activities as needed and percentages will shift as the program evolves.

General Job Requirements (List any required knowledge, skills, experiences or training, including length of time necessary for proficiency, required to perform the job.)

Preferred: Bachelor degree in Social Work, Psychology, Sociology or related field.

A minimum of one-year experience with the immigrant Latinx population providing case management

Bicultural and bilingual English/Spanish recommended.

Ability to work well both independently and as a team member.

Ability to work weekends and evenings.

Salary: $45,000 - $50,000 a year. Hourly rate: $21.63-$20.04

Time off: PTO 20, 4 Mental Health days, and all Federal Holidays observed

Supervision: 1.5 hour weekly Clinical Supervision, 1 hour administrative supervision two times per month, and 1 hour group peer supervision per month. Check in as needed. Reflective supervision offered.

Modalities/Professional Development: Dialectical Behavioral Therapy (DBT), Eye Movement Desensitization and Reprocessing (EMDR), and Somatic Experiencing internal professional development. In addition, $300 stipend for professional development.
