Transitional Care Management's Nursing Career Path Program provides hands-on, specialized learning and mentoring opportunities for Registered Nurses (RN) and Licensed Practical Nurses (LPN). You will be hired as a nurse at of the following locations:
- Bourbonnais - a Specialized Mental Health Rehab Facility
- Chicago - a large inner city Skilled Nursing Facility
- Crestwood - an Intermediate Care Facility
- Frankfort - an Intermediate Care Facility
- Freeport - a Skilled Nursing Facility
- Joliet - an Intermediate Care Facility
- Waukegan - a Skilled Nursing Facility
- West Chicago - an Intermediate Care Facility
As you are working as a Floor Nurse in one of our centers, you will be receiving additional hands-on training in the following sub-specializations:
- Wound Care
- Restorative Care
- Infection Prevention (ICP)
- Nursing Management
- Clinical Reimbursement (MDS)
- Clinical Information Technology (EMR)
NOTE: This training will occasionally require floating to other centers and our corporate office in Lisle to attend training sessions.
We train you while you are working. At the end of one year, you can earn a promotion into one of the above specialty areas/positions.
What we need from you:
- A one year commitment, at full time status
- Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in IL
- Prior Floor Nurse experience within a skilled nursing facility is helpful, but not required
- New Grads Welcome
- Electronic medical records experience is helpful, but not required
- We follow the CMS & IDPH rules for the Covid-19 vaccine. You should be fully vaccinated or you will be required to submit an exemption. Details will be discussed during your interview.

This center is managed by TRANSITIONAL CARE MANAGEMENT. We currently provide comprehensive management services to several inpatient healthcare centers throughout Northern IL & the Chicagoland area. These settings include Long-term Care Skilled Nursing (SNFs), Short-term Care Rehabilitation, Intermediate Care (ICFs), and Behavioral Health Service (SMHRFs) centers.
