The Population Health Clinical Care Coordinator is responsible for coordinating multiple aspects of care for patients with preventative care needs, chronic diseases, and care opportunities within the Providence Medical Group NW region. Daily work includes ordering laboratory tests or imaging exams per protocol to facilitate patient screenings, management of preventative care, and supporting optimization of chronic disease treatments as prioritized thru Enterprise Clinical governance. The Population Health Clinical Coordinator assures that an integrated approach to the patients' care coordination is fulfilled. This position is the liaison between Providence Medical Group NW providers, primary care clinics, specialist offices, patients, and services for population health programs and related processes.

Required Qualifications:

  • H.S. Diploma or GED
  • 1 year for Certified Medical Assistant, a minimum of 1 year of medical assisting work experience in primary/specialty care
  • 1 year for Certified Medical Assistant, a minimum of 1 years of medical assisting work experience in primary/specialty care

Preferred Qualifications:

  • Bachelor's Degree
  • Completion of an accredited Medical Assistant program and registered with State Board (where required by state) and/or certified through a nationally recognized Medical Assistant program such as AAMA, AMT, RMA, CCMA or NCCT upon hire or
  • Nationally Certified Pharmacy Technician or registered with the State Board upon hire

About the Team

Providence Physician Enterprise (PE) is a service line within Providence serving patients across seven states with quality, compassionate, coordinated care. Collectively, our medical groups and affiliate practices are the third largest group in the country with over 11,000 providers, 900 clinics and 30,000 caregivers.
