Job Description
Asian Community Care Management, Inc. is looking for a Chinese Bilingual Registered Nurse/Care manager or a Social Worker/Care manager to provide telephonic coordination care for members through assessment and member-centered care planning. This is a 100% remote position.

Requirements:
- Bachelor degree in Social Worker or Registered Nurse
- RN License required if you apply for a RN position.
- License Master Social Worker Preferred if you apply for a SW position,
- 1 year+ Homecare Experience Preferred
- 1 year+ Case Management Experience Preferred
