We are looking for Full time and Part Time professional candidates who are self-motivated and organized to manage a caseload of Child HCBS (Home & Community Based Services). This position provides a family-driven, youth guided model of care that emphasizes and supports a child/youth's potential by optimizing quality of life and reducing symptoms of mental illness and substance use disorders.
Services are provided to a person in their own home or within their community. Services follow these core principles: Person-centered, recovery-oriented, data-driven, evidence-based, trauma-informed, culturally competent, flexible and mobile, inclusive of social networks and should be provided with coordination and collaboration of natural supports and other service providers.
Requirements
Education and Experience
- Associate's Degree and1 year relevant experience working with children/youth OR 1 year relevant experience working with individuals with substance abuse disorders and/or SMI.
- Candidates with Bachelor's Degree in relevant field and with intern experience are encouraged to apply.
Skills, Knowledge and Abilities
- Knowledge of the Behavioral Health system.
- Must be able to read and write case notes and be able to follow written directions.
- Must be able to manage multiple assigned tasks at one time and meet assigned deadlines.
- Must have basic computer knowledge and must be comfortable using web-based case note, medication administration, time clock and employee portal systems.
