About the Job
Elwyn is looking for a Certified Peer Support Specialist to provide psychiatric rehabilitation and resources as part of our Blended Case Management Program for individuals with serious and persistent mental illness and co-occurring disorder. The Certified Peer Support Specialist works in conjunction with other staff to help facilitate client progress toward recovery-based treatment goals. Peer specialist interventions encompass all client life domains including physical and behavioral wellness, daily living support, relationship building, and vocational skills.
Responsibilities
  • Integrate the PRISMMODEL for the effective treatment of serious and persistent mental illness
  • Maintaining accurate clinical notes reflecting client needs, preferences, and actions
  • Assisting with the development of treatment plans for clients which outline client needs, goals, and measurables
  • Developing clinically informed and therapeutically beneficial relationships with program individuals

Qualifications
  • High School diploma or GED required
  • Peer Specialist certification is required
