NORTH JERSEY RECOVERY CENTER is seeking a dedicated and compassionate evening Case Manager who will also function as a Group Facilitator of psycho-educational groups, completing required documentation. As a Case Manager/Group Facilitator, you will be responsible for coordinating supports for individuals who struggle with mental health and co-occurring disabilities and enhancing the quality of patient management during our evening and or hybrid work schedule (day/evening). The successful candidate will be accountable for the full patient care cycle. Case manager duties include assessing, planning, implementing, monitoring and evaluating actions required to meet the client’s health and human services needed. A certified case manager is a plus. CADC or CPRS preferred.
MANDATORY SCHEDULE: *5:00 PM - 9:00 PM MONDAY, TUESDAY, THURSDAY* (PART-TIME) OR (FULL-TIME) DAYTIME HOURS & ABOVE EVENING SCHEDULE HOURS
Responsibilities:
- Conducting Intakes for new patients.
- Coordinating care with any outside providers that the client has appointments with.
- Maintaining client related data systems, including case notes and complete KIPU entries.
- Preparing case management outcomes including but not limited to discharges and clients leaving against medical advice.
- Completing case management needs assessment of all clients assigned to Case Manager.
- Developing appropriate individual plans with clients as the means for implementing appropriate services.
- Providing on-going supportive and/or case management functions in accordance with the problems, needs, and the strategies identified within the treatment plans and needs assessments to assist the clients in achieving the stated goals and objectives.
- Providing ongoing face to face reviews with the client on a regular basis to assess the progress made in reaching goals so that the treatment plans can be modified as necessary to ensure that the goals and objectives are being achieved.
- Documenting all contacts within 24 hours of contact including face-to-face, collateral and networking contacts.
- Demonstrating appropriate & complete documentation of treatment – related information including aftercare plans and reviews, correspondence, releases of information, etc. in compliance with agency standards and regulations.
- Working with clients to develop individualized case management plans.
- Resolving conflicts through general and supportive advice.
- Preparing educational and motivational lectures.
- Running psycho-educational groups when needed.
- Maintaining the confidentiality of all patient and facility activities.
Skills:
- Previous experience with substance abuse population
- Previous experience with psychological aspects of care
- Effective communication skills, both verbal and written
- Excellent organizational and time management skills
- Knowledge of various therapeutic techniques and interventions
- Ability to work collaboratively as part of a multidisciplinary team
- Proficiency in conducting assessments and developing treatment plans
- Familiarity with medical terminology and psychopharmacology is a plus
Requirements:
- Bachelor's degree in social work, psychology, or a related field (Master's degree preferred)
- CADC or CPRS preferred
