Overview of Enhanced Care Management (ECM)
Enhanced Care Management (ECM) Program is a community-based, interdisciplinary, high-touch program intended for high-risk Medi-Cal managed care members with the most complex medical and social needs. ECM will provide these members with person-center support in coordinating their services across delivery systems to successfully address their needs.
Position Summary:
The Case Manager, also known as the Lead Care Manager (LCM) operates as part of the member?s multi-disciplinary care team and is responsible for coordinating all aspects of ECM. Under the direction of the ECM Director and/or Clinical Consultant, the LCM will assist in the development of treatment plan(s) that are appropriate to the needs and desires of the consumer. The treatment plan(s) will aim to promote recovery and stability for community living and helping the client to achieve higher levels of function. This position is the primary care manager for the ECM member and will be responsible for coordinating with those individuals and/or entities to ensure a seamless experience for the member and non-duplication of services.
Core Duties And Responsibilities:
- Oversee provision of ECM services including outreach and engagement.
- Provide person-centered intensive case-management services and developing care plan.
- Offer services where the member lives, seeks care, or finds most easily accessible and within ECM guidelines.
- Connect member to other social services and provides supports, including transportation.
- Advocate on behalf of members with health care professionals.
- Use motivational interviewing, trauma-informed care, and harm-reduction approaches.
- Coordinate with hospital staff on discharge plan.
- Accompany member to office visits, as needed and according to ECM guidelines.
- Monitor treatment adherence (including medication).
- Provide health promotion and self-management training.
- Participate in the Mental Health education program provided to the community.
- Coordinate client care with other providers both within and outside the agency.
- Attend and participate in all in-service training as necessary.
- Responsible for timely completion of documentation in the client?s clinical record per Enki, LA County Department of Mental Health and legal requirements.
- Responsible for meeting assigned minimum productivity requirements on a monthly basis.
- Other duties as assigned.
- Comply with all Company policies and procedures as applicable.
Minimum Education And Experience Preferred:
- High school diploma plus two (2) years experience in a mental health setting; OR Bachelor?s Degree from accredited college/university in a related field (i.e. social work, psychology or sociology).
Licenses/Certifications:
- Valid California driver?s license, current automobile insurance, and eligibility to drive for the Company.
Knowledge, Skills And Abilities:
- Ability to perform in a positive, courteous, flexible and adaptive manner.
- Able to exercise independent judgment and initiative.
- Computer literate and experienced with computer programs such as Microsoft Office Word, Outlook and Excel.
- Experience using an Electronic Medical Health Record databases is a plus.
- Must be able to communicate clearly, both orally and written.
- Bilingual English/Spanish preferred but not required.
Physical Requirements:
- See well enough (with or without corrected vision) to read fine print, hand-written materials, and a computer screen.
- Speak and hear well enough (with or without assistive device) to converse in person and by telephone.
- Finger, handle, feel, and reach with hands and arms, including ability to use a computer keyboard.
- Have the mobility (with or without assistive device) to stand, sit, crouch, and move from one work area to another.
- Have the strength to lift, carry or push office equipment and supplies used in normal job functions up to 20 lbs.
- Stamina to effectively perform work activities.
Pay Range: $25.00 - $28.00/Hour (Determined by Experience & Qualifications)
