Program Summary:The Suicide Prevention Program for Adolescents and Young Adults is a research-based program that provides care coordination, family education and treatment services to youth and families experiencing suicidal thinking or behavior. The program provides case management services, including care coordination with therapy and school, life skills groups, individual and group therapy, and a 12-week psychoeducational program that trains nominated support persons in Suicide Prevention and to support a young person in treatment. Caregivers, families, school staff, and emergency room personnel can refer a young person to the program. The program runs alongside the traditional care for adolescents with a history of suicidal thoughts or suicide attempts which can include medication, psychotherapy, group intervention, or partial hospitalization.

Position Summary:The role of the Case Manageris to work as part of a multidisciplinary team that provides services designed to assist and support an individual diagnosed with or being assessed for a mental health disorder, to gain access to needed medical, social, educational, and other services as well as provide care coordination and care transition services.

The Case Manager provides evidence-based interventions to the individual and support system to help them gain access to needed medical, social, educational, and other services as well as provide care coordination and care transition services.

We are seeking energetic, creative, and compassionate individuals to fill our open Case Manager position with the Suicide Prevention Program. Our desired applicant will have their bachelor's degree or are interested in/working towards a master's degree in one of the recognized mental health professions. Our team supports employees working towards their master's degree find an internal internship to get their clinical degree requirements met. Experience in emergency services, behavioral health, integrated care, Suicide Prevention, evidence-based interventions such as Motivational Interviewing or Dialectical Behavioral Therapy, crisis intervention, and coordination of care or demonstrated competency preferred.

We take a team-based approach to treatment and provide peer and direct supervision on cases. All our staff receive individual supervision and consultation for support, guidance, and professional development. Our staff are offered many ongoing training opportunities to increase knowledge and skills in a variety of areas that impact our work on this team including but not limited to Suicide Prevention, trauma informed approaches, family-focused interventions, child and adolescent specific interventions, and cultural competence and diversity.

Salary: $21.00 (Base pay $18.50 + $2.50 locale differential = $21.00) - $23.72 per hour and considers preferred experience and preferred education, as described above.

Physical demands:Employee must occasionally lift and/or move up to 15 pounds. Employee is frequently required to walk, sit, stand, or kneel and occasionally required to climb or balance and stoop. Must have ability to sit for longer periods at a computer. Employee must be able to travel between AHS locations. Possible potential exposure to communicable disease. The noise level in the work environment is usually moderate. AHS may make reasonable accommodations to enable individuals with disabilities to perform the essential functions.

Required Skills:

  • A working knowledge of crisis intervention approaches and their applications to clients with emotional, behavioral, and substance abuse problems.
  • Working knowledge of Brief Solution Focused Therapy, Motivational Interviewing and DBT coaching skills.
  • Ability to perform mental status evaluations, differential diagnosis, and service coordination duties.
  • Ability to assess level of risk and treatment approaches and their applications to clients with emotional, behavioral, and substance abuse problems. Make appropriate clinical judgment for resolution of client emergencies to a safe outcome.
  • Ability to deliver services in various community locations.
  • Working knowledge of involuntary mental health holds (27-65), and alcohol and drug Emergency Commitments (EC) and Involuntary Commitments (IC).
  • Working knowledge of HIPAA and 42 CFR, Pt. 2.
  • Knowledge of community health and behavioral health resources and their eligibility criteria.
  • Working knowledge of Windows based computer applications to include MS Word, MS Excel, MS Outlook, and Internet access.
  • Ability to operate standard office equipment including calculator, copy machine, fax and multi-line telephone. Interpersonal and communication skills to develop and maintain effective working relationships with all internal and external customers.
  • Ability to organize and prioritize workload in a sometimes-hectic environment with frequent interruptions.
  • Ability to follow established universal precautions and report potential exposure to a communicable disease immediately.
  • Must have current driver's license and ability to be commercially insured and maintain a driving record that allows insurability with AHS commercial insurance agency.
  • Travel required.
  • Ability to take on additional duties when necessary.

Additional eligibility requirements:Annual Flu immunization, Annual TB screening, CPI training within 90 days of hire, BLS PRO certification required within 90 days of hire.
