The Healing Center of Mass, a leading outpatient substance abuse treatment facility, is seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will be responsible for working with clients to develop individualized treatment plans and coordinating social services to help them reintegrate back into society as healthy and productive members.
Responsibilities:
- Meet with clients upon admission and conduct a comprehensive assessment to identify their individual needs and goals
- Develop and implement individualized treatment plans in collaboration with the clinical team
- Coordinate social services for clients, including but not limited to, food stamps, cash assistance, FMLA, disability, resume building, job searches, and other assistance with integration back into society
- Provide ongoing support to clients, including monitoring their progress and adjusting treatment plans as necessary
- Maintain accurate and timely records of client progress and social service coordination efforts
- Participate in case conferences with the clinical team to ensure coordinated and effective treatment
- Collaborate with community agencies and resources to connect clients with appropriate services
Requirements:
- Bachelor's or Master's degree in social work, psychology, or a related field
- Minimum of 2 years of experience in case management or social work, preferably in a substance abuse treatment setting
- Strong organizational and time management skills
- Excellent interpersonal and communication skills
