Case Manager-Supervised Release & Outpatient Competency Restoration Program-17 County Territory

Due to program expansion, ACC Community Connections is recruiting for several career minded individuals for the position of Case Manager-Supervised Release & Outpatient Competency Restoration Program

 

The counties ACC Community Connections currently serves include: Portage, Waupaca, Outagamie, Winnebago, Fond du Lac, Adams, Wautoma, Marquette, Columbia, Dodge, Green Lake, Calumet, Manitowoc, Kewanee, Sheboygan, Washington, and Ozaukee.


Job Purpose

This professional position is responsible for a broad range of community-based case management services for individuals committed to the State of Wisconsin Department of Health Services under Chapter 980, Supervised Release (SR), as well as those participating Outpatient Competency Restoration Program (OCRP). The services are grounded in the dual philosophies of community safety and best practice recovery methods.

Job Responsibilities

The primary duties and responsibilities are as follows:

  • Provide regular, timely, and professional contact with clients to monitor progress/recovery, rule compliance, symptom management, medication compliance, and overall stability in the community.
  • Know and follow program specific guidelines and contract expectations for Supervised Release (SR) and Outpatient Competency Restoration Program (OCRP).
  • Coordinate, plan, and ensure follow through with community/clinical services to meet the needs of clients in a cost-effective manner. Targeted areas for assistance include development of detailed schedules for SR clients, money management, vocational/educational pursuits, scheduling appointments, providing or arranging for transport to appointments.
  • Facilitate treatment team(s) collaboration to best meet client recovery outcomes.
  • Use Motivational Interviewing skills as a primary collaborative conversational skill/tool to strengthen a client's own motivation for and commitment to change. Commit to improving Motivational Interviewing skills.
  • Assist clients to develop a natural support system which can include family, friends, neighbors, community resources, etc.
  • Manage crisis situations with appropriate interventions while working collaboratively with other individuals on the client’s treatment team.
  • Prepare all client service documents in a timely manner. Forward documents to Director of Case Management for approval, makes recommended changes.
  • Testify in court being professional in appearance and thoroughly prepared.
  • Maintain a complex calendar for each assigned client.
  • Comply with all company and DHS quality standards and time frames for all documents and communications.
  • Be very familiar with community resources and seek further development based on client and agency needs.
  • Prepare approved residences for occupancy prior to client being released into the community.
  • Collaborate with Director of Case Management, DHS contract specialists, probation agents and other necessary individuals regularly but especially in difficult case issues, high profile people, and/or events.
  • Attend and participate in case manager meetings.
  • Work with case management team.
  • Executes the Principles of Trauma Informed Care with clients and colleagues within the agency. Strives to enhance knowledge and abilities relating to Trauma Informed Care. Participates in personal development of skills and knowledge of Trauma Informed Care. Supports the agency’s perpetual development of improved practices and processes.
  • Maintain strong professional and ethical boundaries.
  • Regularly contribute to the development of and implementation of program policies, procedures, and quality assurance activities.
  • Work as a team player with peers and client teams.
  • Other duties identified and assigned.

 

Qualifications

Education/Experience:


  • Bachelor’s degree in Human Services or related field.
  • Strong organizational, planning, project management, problem resolution, communication, presentation, facilitation, and influencing skills required as are strong use of ethics and boundaries.
  • Experience with Microsoft Office, Excel, and PowerPoint as well as visual aids technology.
  • Must have a valid driver’s license and access to an insured vehicle. Must have a willingness to transport clients from all programs in company vehicles.
  • Must be able to pass a criminal background check as set forth by the Wisconsin Department of Health Services, Department of Corrections as well as a Wisconsin Caregiver Background Check and out of state background check if applicable.

  • Advanced organizational skills with the ability to handle multiple assignments.

Professional Competencies

  • A foundational understanding of Wisconsin State Statute 980 along with the OCR Program along

with a willingness to increase knowledge base.

  • Knowledge of clinical assessments, diagnosis, planning and intervention skills, standards for those with mental illness, AODA, and co-occurring diagnoses.
  • Knowledge of best practice standards in the area of services to persons living with mental illness, substance use, and or co-occurring diagnoses.
  • Knowledge of and support for principles of Trauma Informed Care, Recovery, & Client Centered
  • Ability to communicate effectively orally and in writing; the ability to effectively exchange information with various stakeholders in a manner that makes meaningful results possible.
  • Ability to establish and maintain effective working relationships with clients, administrative superiors, colleagues, the general public, and community partners in a manner that supports collaboration.
  • Ability to collect, organize, and utilize data for program improvement purposes.

Work Environment

 

The work is performed in home office settings. There will be extensive travel to various designated locations in order to meet with the clients. Flexible work hours are within the parameters of Monday – Friday, 7:00am – 6:00 pm. However, there may be occasions to adjust the schedule to accommodate client needs.

 

The work is primarily sedentary; there is no heavy lifting, excessive standing, or bending.  Must have the ability to drive a car. Case managers drive clients in a company owned vehicle.
