We're looking for passionate individuals to join our team as Outreach Liaisons. This role is all about connecting with patients directly, both in-person and over the phone, to increase their engagement with our services. You'll also be building meaningful relationships with community partners and spreading the word about the positive impact of Author Health.

Your Role: Making Connections That Matter

Be the Hero: Respond to Health Information Exchange alerts by meeting with patients and their caregivers in various settings, such as hospitals or their homes.

Bridge Builder: Work closely with local leaders and organizations to broaden our outreach and make sure more people can benefit from our services.

Knowledge Sharer: Educate health systems, PCP groups, and community organizations about the value of what we do.

Goal Achiever: Meet and exceed monthly engagement targets through dedicated outreach and effective communication.

Team Player: Collaborate with Market Managers and the partner success team to identify key areas and strategies for outreach.

What We Need From You:

• Strong organizational skills and excellent verbal/written communication abilities.
• A valid driver’s license and reliable transportation.
• At least 1 year of experience in a role related to these duties.
• A passion for working with senior patients dealing with behavioral health conditions.
• Knowledge of community resources and a knack for connecting people with the help they need.
• Willingness to travel for outreach activities.

Preferred:

• Associate's or Bachelor's degree in health/human services or other relevant field preferred
• Direct-to-consumer sales or Patient Engagement at a health plan, medical facility or another healthcare setting
