Seeking an experienced Case Managerwho will be responsible for helping CEO manage new business demand by conducting Intakes/Assessments for new clients and following them throughout the continuum of care. Your role will be to ensure our caregiving team delivers quality care and that our clients are happy they chose our company.
Case Manager Functions
- Assesses new client needs
- Creates Care Plan to be executed by Care Team members (caregivers)
- Conduct introductions of Care Team Members to clients on first day of service
- Coordinates with Care Team, Care Coordination Team and other team members to ensure the delivery of high-quality Home Care
- As ears/eyes in the field, provides feedback to CEO and office team to refine delivery of quality care
- Serves as a client and care team member advocate, facilitates a collaborative relationship between client/family and our agency, maximizes clients/familys ability to make informed decisioni
- Certification in Case Management would be advantageous, but not required
- Two years of experience in healthcare setting preferred
- Prior experience in case management preferred
- Caring and compassionate personality required
- Teamwork mentality is a must
- Ability to see the big picture and distill into an executable Care Plan for client
