Registered Nurse Case Manager-Hospice
Duties:
- Provide comprehensive case management services to patients
- Assess patient needs and develop individualized care plans
- Coordinate and facilitate communication between patients, families, and healthcare providers
- Collaborate with interdisciplinary teams to ensure continuity of care
- Conduct discharge planning and coordinate post-discharge services
- Monitor patient progress and adjust care plans as needed
- Utilize utilization management techniques to optimize patient outcomes and resource utilization
- Provide patient education on disease management, medication adherence, and self-care techniques
#### Skills:
- Strong knowledge of physiology and medical terminology
- Experience in pediatrics, behavioral health, or acute care settings preferred
- Proficient in case management principles and practices
- Excellent communication and interpersonal skills
- Ability to prioritize and multitask in a fast-paced environment
- Familiarity with managed care systems and insurance requirements
