Job Title: Flex Case Manager (Independent Contractor)

Minimum Requirements:

• Must reside in Minnesota. Be willing to travel and work as an Independent Contractor (IC), work from home and communicate virtually.
• Pick how many hours a day you want to commit to working (minimum 3 and max of 8 hours).
• Provide proof and maintain coverage for Professional Liability Insurance.
• Willing to follow all public COVID-19 guidelines while visiting facilities and/or in the community with clients.
• Bachelor’s Degree in a related social service field, along with 2 years of experience in providing direct social services (references required).
• Valid Driver’s License and continual proof of current insurance coverage.
• Advanced skills in navigating computer systems (Word, Excel, Google Suite and other Internet databases).
• Be flexible with time management skills; work well independently and the ability to collaborate with a team.

Job Description:

• Communicate effectively, positively and professionally with service participants, providers, family members and others, often from diverse backgrounds.
• Conduct housing searches and perform non-traditional tasks while meeting client needs and be a self-starter that is motivated to learn.
• Complete client-related tasks as directed by BLRS.
• Complete case notes daily, therefore, have the ability to write professionally; must be able to to begin documenting all assigned tasks upon hire with minimal support.
