Case Manager Assistant

Job Summary: Engage individuals 50+ living with HIV that are facing age-related illness and co-morbidities with the goal of improved health outcomes.This position will provide specific People Aging with HIV (PAWH) grant services (technology coaching, health education, population specific cognitive/behavior screenings) to assigned duly enrolled PAWH/Health Home or Ryan White clients. This position will also assist Health Home Care Coordination or Ryan White Case Management teams by providing training and technical assistance to build capacity to address the unique needs of those aging with HIV.

Major Activities:

• Incorporate best practices regarding confidentiality into all job duties and communications in accordance with HIPAA, Article 27-F, Alliance for Positive Health policies and procedures and other applicable regulations. Observe and abide by HIPAA and the HIV Confidentiality Law.
• Conduct trainings and provide technical assistance to Health Home Care Coordination and Ryan White Case Management staff on addressing the unique needs of those aging with HIV.
• Complete required tasks and documentation in the AWARDS electronic health record (EHR) by required deadlines, as assigned by the Program Manager to include:
  • Intakes, PAWH-specific assessments and reassessments, health education screenings, coordinated care planning, etc.
  • Assist in maintaining contact within the interdisciplinary team of Health Home/Ryan White and PAWH staff members.
  • Coordinate and attend monthly case conferences with Health Home/Ryan White staff members.
  • Ensure all duly enrolled clients have their most recent Health Home/Ryan White assessment and care plans on file within the AWARDS EHR.
  • Rotate through all regional offices to serve clients, conduct psychosocial support groups, conduct outreach, and connect with local providers.
  • Conduct home and field visits, as needed.
  • Escort clients to appointments as needed.
  • Track patient medical appointments, labs and other activities
  • Assist with facilitating health education sessions to clients both individually and as part of a group.
• Provide information to clients about disease management, medications and treatment adherence, psychosocial issues and harm reduction education.
• Incorporate the Transtheoretical Model of Behavior Change, as applicable, into all aspect of programming.
• Perform other duties as assigned.

Qualifications:

• Associate degree in social work or related discipline with one-year experience working with persons with HIV/AIDS, a history of mental illness, homelessness, chemical dependency, chronic medical conditions, and/or other populations of persons in need.
• Must have a valid, unrestricted driver’s license and independent means of transportation for frequent travel throughout catchment area.

Employment Classification:Non-exempt;full-time 37.5 hours per week. Position range is $18.50-19.25/hour dependent on qualifications and experience.
