Specific Duties of the ADULT FOSTER CARE (AFC) Care Manager.
- Conduct an initial psychosocial assessment, home assessment, and evaluate the appropriateness of participation in the program.
- Evaluate, support and train the caregiver
- Participate in developing and ensuring implementation of the plan of care
- Conducts home visits
- For Level I. bimonthly alternating with RN
- For Level II, monthly Home visits
- Review and document fire and safety procedures for AFC program homes
- Review caregiver daily notes on a regular basis
- Complete a care management progress note for each home visit, all communication for the case and upon significant change
- Conduct regular, periodic assessments, at least annually to ensure that each home meets the requirements for qualification.
Responsibilities:
- Provide case management services to individuals and families in need
- Conduct assessments to determine client needs and develop appropriate service plans
- Collaborate with other professionals and agencies to coordinate services and resources
- Advocate for clients and ensure their rights are protected
- Monitor client progress and make necessary adjustments to service plans
- Provide crisis intervention and support during difficult situations
- Maintain accurate and up-to-date case records
Skills:
- Strong knowledge of child welfare policies and procedures
- Experience working with children and families in various settings
- Familiarity with childcare regulations and best practices
- Understanding of child protective services and reporting requirements
- Knowledge of social work principles and practices
- Ability to work with individuals with developmental disabilities
- Excellent communication and interpersonal skills
- Strong organizational and time management abilities
