Flourishing Community Initiative (FCI) is a Sunshine Gospel Ministries (SGM) program. Through FCI we envision a community that inspires youth and residents to live with pride and to live in harmony with purpose with one another. We envision the sounds of gunshots replaced by the sounds of organizing, collaboration, laughter, and learning.
POSITION SUMMARY
The Case Manager works to provide linkages and robust accompaniment to pro-social alternatives including jobs, workforce education, and many other social services. The Case Manager's position is a full-time, 40 hours a week, non-exempt position. The Case Manager will work with individuals and families who have been directly affected by Gun Shot Wounds (GSW), including death, or secondarily by this street violence to provide support and advocacy. This role is also accountable for assessing the needs and ensuring the proper referrals for counseling services and job training. The Case Manager will work with individuals or families in coordination with our Outreach and Victim Advocacy partner teams.
RESPONSIBILITIES:
- Engages participants in interviews to assess specific needs and determine need for additional services and referrals.
- Develops relationships with partner organizations.
- Identifies pathways to benefit potential clients.
- Coordinates internal and external services being provided to participants; identifies and arranges resources.
- Facilitates connections between service providers and participants by attending initial sessions and problem-solving barriers for service accessibility.
- Provides direct case management and emergency support services to victims of GSW violence/trauma and their families through observations, in-person visits, case notes, case reviews, phone calls, criminal justice advocacy, and referrals to other support organizations.
- Monitors cases by verifying participants' attendance; observing and evaluating activities and responses.
- Manages “case management” documentation and reporting, including preparation and submission of monthly reports. Enters program information into appropriate data tracking systems to support tracking and evaluation.
- Communicates participants' progress by engaging in weekly interdisciplinary meetings and evaluations, disseminating results and obstacles to team and family, and identifying program influences.

RELATED DUTIES:
- When needed, position may require evenings and weekends, including holidays with a flexible work schedule.
- When needed, position may require visits at Juvenile Detention Center.
EDUCATION SKILLS
- Bachelor's degree in Social Work, Psychology, Criminal Justice or related field.
- Three years of experience in case management responsibility is essential.
- Must become a Mandated Reporter.
- Must become HIPPA (Health Insurance Portability and Accountability Act) certified.
- Must become HAVI (Health Alliance for Violence Intervention) Compliant.
- Demonstrated understanding of the impact of trauma on individuals and experience with trauma-informed practices and programming is essential.
- Self-motivated with the demonstrated ability to set meaningful goals and take action.
- Demonstrated critical thinking with analytical and problem-solving skills and be detailed oriented.
- Continuous learner who leverages learning opportunities and applies new knowledge and skills.
- Has experience developing and retaining relationships with law enforcement, community stakeholders, and other service providers.
- Must be punctual, organized, flexible, and effectively manage time and projects well.
- Ability to use discretion and judgment when dealing with confidential information.
- Capability of making good decisions under pressure
- Must possess excellent oral and written communication skills.
- Must have a valid Illinois Driver's License, insurance, and a good driving record.
PREFERRED:
- Has at least 1-year experience supporting and working with the at-risk community.
- Has at least 1-year experience working with people from various ethnic, economic, and educational backgrounds both as colleagues and participants.
- Has experience documenting case notes.
