Job Summary:
We are seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will be responsible for providing support and guidance to individuals and families in need. Your role will involve assessing client needs, developing care plans, coordinating services, and advocating for clients. The ideal candidate will have experience in motivational interviewing, crisis management, addiction counseling, and child welfare.
Duties:
- Conduct comprehensive assessments to determine client needs and develop individualized care plans
- Provide counseling and support services to clients, including addiction counseling and crisis intervention
- Coordinate services and referrals to ensure clients have access to necessary resources and support systems
- Collaborate with other professionals, such as healthcare providers and social workers, to ensure holistic care for clients
- Advocate for clients' rights and needs within the community and with government agencies
- Monitor client progress and adjust care plans as needed
- Provide mentoring and guidance to clients in areas such as workforce development and childcare
Requirements:
- Bachelor's degree in social work or related field (Master's degree preferred)
- Proven experience in case management or a related field
- Knowledge of community resources and services
- Familiarity with Section 8 housing programs and Medicare regulations
- Strong communication skills, both written and verbal
- Ability to work independently and as part of a team
- Excellent organizational skills with the ability to prioritize tasks
- Compassionate and empathetic approach to working with individuals in crisis
