Yoursocial workeror case manager acts as a client advocate, educator, advisor, and coordinator of care.Care coordination includes services such as: home health care, medical equipment rentals, transportation to follow up provider visits, and other related activities. This might often include referrals to inpatient and outpatient services, funding sources, and community support resources for families.The services of the social work and Case Management department are included with your stay at Springfield Hospital and are a resource for you and your family.Case Manager will:Promote patients' health maintenance and restoration to provide quality, cost-effective outcomesInitiate a discharge plan to meet immediate post hospital needs, as well as visualizing long term needs, to optimize health maintenance and prevent readmissionEducate patients, family and staffCollect, analyze, and interpret data to evaluate current processes, and alter practices that don't achieve optimal patient care and customer serviceSeek learning opportunities to expand current clinical knowledgeSeek learning opportunities to expand cost containment strategiesRequested time off during the course of this assignment MUST be noted and verified at upload.RTO REQUESTS AFTER UPLOAD WILL NOT BE APPROVED!!NO MORE THAN 5 DAYS OF RTO WILL BE ACCEPTED (less is better).Travelermustbe willing to work holidays, including weekends before(If Needed/Scheduled).
