MAKE A DIFFERENCE AS A CASE MANAGER
As a key player in our team, you are at the forefront of providing comprehensive care management, ensuring individuals receive personalized, person-centered care plans. Your role includes coordinating and facilitating physical health treatment plans, reducing barriers to adherence to medical and behavioral health appointments, and proactively reaching out to primary care and behavioral health providers. You serve as a lifeline to the community, identifying and connecting patients with essential social supports, such as food, shelter, employment, and legal resources, while fostering strong partnerships with community organizations.
Additionally, your in-person support extends to patients and their families, assisting with treatment and medication adherence, offering caregiver counseling and training, and identifying valuable resources to enhance their well-being. In crisis situations, you provide vital transitional care and participate in developing discharge plans, ensuring seamless access to follow-up care post-discharge. Your dedication to health promotion includes educating patients and families on independent living skills and empowering them with attainable goals to achieve their aspirations. It's a rewarding role with a profound impact on individuals' lives.
QUALIFICATIONS
The knowledge and skills required for the position are:
- Bachelor's degree in a health-related field of counseling, psychology, social work, or other behavioral sciences
2+ years of experience in a behavioral health position is preferred, though not required.
