Job description

Conducts thorough and comprehensive Case Management assessments that identify the individual strengths and needs of each client. Synthesizes assessment information to prioritize problems that can be addressed by available services and resources. Coordinates and collaborates with clients and the therapist to develop personalized, achievable goals, objectives and interventions that promote self-reliance.
ESSENTIAL FUNCTIONS:

6. Meets contractual obligations for the program that allow the program and organization to legally bill for services.
7. Productivity standards as established by the organization are consistent with documented billable units of services or other established criteria.
8. Productivity is consistent with accomplishment of designated job duties and meets organizations productivity standards.
9. Work is consistent with all laws, policies, regulations, and ethical guidelines as outlined in compliance training.
10. Identifies and reports any compliance violation.
11. Completes, and is current with compliance training guidelines.
12. Practices attentive and active listening; uses language and behavior to promote dignity and respect; develops effective partnerships with others to solve problems, resolve conflicts and achieve goals.
13. Works to promote the collective effort and cooperation of team/committee members to identify and achieve common goals.
14. Shares information, proposes ideas, and provides feedback to enhance performance.
15. Communicates and collaborates with internal and external providers to ensure continuity of services and resources for intended recipients.
16. Recognizes and acknowledges the impact of different experiences, cultural backgrounds and lifestyles encountered in the workplace and the community.

18. Plans for and provides effective, understandable, and respectful service to both staff and consumers that are compatible with an individual’s beliefs.

21. Travels into the community to assess, engage, support, and deliver services to clients.
22. Provides information about, parenting skills, coping strategies and community resources so that clients can increase their ability to manage basic needs and make informed decisions.
23. Provides direct assistance in meeting basic needs for crisis intervention, food, clothing, shelter, financial support, and/or health care when clients are unable to accomplish this on their own.
24. Provides linkage, monitoring/follow-up, referral, advocacy and crisis intervention and sufficient follow-up to ensure that clients are connected to needed services and supports both within and outside the organization.
25. Teaches simple and complex skills including physical, social, cognitive, and emotional skills that leads to improved client functioning in their living, working, learning and/or social environment.
26. Uses interpersonal skills to engage and support clients as they deal with issues such as problem solving and goal attainment.
27. Generates accurate and timely documentation of service provision; provides accurate and timely summaries of client progress and/or service activities, which results in high quality records meeting all applicable standards and procedures.
28. Responsible for completing competency-based workplace safety training as assigned by the agency.
29. Other duties as assigned
