Mission:
Vista Del Mar provides a trauma-responsive continuum of services to empower children, youth, and families in Southern California to lead fulfilling lives.
Role Overview:
- The Case Manager is responsible for coordinating case management activities through participation in the Office of Refugee Resettlement (ORR). The Case Manager provides trauma-informed, individualized case management services according to ORR and other regulatory requirements. The Case Manager works closely with the Federal Field Specialist, Case Coordinator, Contracted Field Specialist, and The Providencia Group to vet all identified sponsors for the resident for the purposes of family reunification. The CM documents services and collects evaluative data. Coordinates release to the sponsors and ensures the unaccompanied residents safety.
Qualifications:
- Bachelor's degree or equivalent work experience will be considered
- 2 years related experience minimum
- Bilingual ANY language
- Knowledgeable regarding state and county regulations for residential care as well as patient's rights-related issues.
- Ability to hear and verbally communicate with others.
- Must be able to learn/use necessary computer programs
Essential Job Duties:
- Assist residents in obtaining personal documents such as identification, legal documentation, and other relevant information to complete the vetting of the sponsor and prove a relationship with the resident.
- Expected to have contact with residents on their caseload at least once a week or as needed to establish and assess progress toward short-term goals, such as extracurricular activities, medical needs, mental health resources, transitional support, and ongoing long-term planning.
- Responsible for documenting these interventions as per ORR standards and meeting requirements to update the UC Portal, case note all engagements regarding all entities spoken to in the case, update the case review (weekly), complete assigned assessments, and update the ISP's, when applicable.
- Submits the Notice of Placements and Evidentiary Records for transfer youth (in step up or step down).
- Communicates all case-related updates to the FFS, CC, CFS, and TPG on an as-needed basis and seeks support from these entities to move forward with case approvals.
- In coordination with the Lead Case Manager and Clinician make appropriate referrals and linkages to services for residents and their families/identified sponsors including recreational activities, co-facilitate group therapy, specialized groups, mental health services, transitional services, medical support, etc.
- Weekly communicates with the YDC team members to assess the case's progress and retrieve resident updates of their day-to-day function and collaborate with the YDC team and the Clinician to adjust services and/or goals as needed.
- In conjunction with the clinician, coordinate discharge planning tasks.
- Accurate and timely completion of assigned ORR documentation related to care and custody timeframes.
- Coordinates meetings and makes recommendations with the resident's Child Advocate and Legal Service Provider.
- Attends and participates in school meetings, team huddles, family sessions, cottage meetings, and case staffing.
- Participates in family sessions and or family therapy sessions to track accountability for tasks and activities assigned to complete the FRA and sponsor vetting.
- Completes assessment of the sponsor. Completes Significant Incident Reports (SIR's) to document disclosed incidents impacting the health and safety of the resident.
- Updates caseload trackers and submits data points in a timely manner to ensure case organization. Completes specialized training related to the case management role as assigned by ORR and CCL.
- Utilizes strengths-based, trauma-informed, relationship-based, culturally responsive, and client and family-centered approaches.
- Coordinates travel for family reunification. Upon case approval and discharge, CM travels for the purposes of family reunification with the resident.
