Astera Health currently has a Part-Time Case Management/Discharge Planning Registered Nurse (RN) opening at the Wadena campus. The primary responsibilities for this role is to provide assessment, planning, implementation, coordination, evaluation, and monitoring of patients for health options and services from pre-admission to post-discharge.
Schedule/Hours Available:
- Part-Time: 32 hours per week, 8-hour day shifts, 8:00am-4:30pm

Some responsibilities of the Case Management/Discharge Planning RN include, but are not limited to the following:
- Assist in the creation of an individualized discharge plan which includes the patient’s family members and health care team to aid the patient in their transition from one level of care to another.
- Analyze patient records to determine medical necessity for admission, treatments, and length of stay through daily utilization review.
- Prepare clinical appeals, as appropriate, on medical necessity, level of care, length of stay, and authorization denials for hospitalized patients.
- Provide clinical insight and education to nurses and providers related to medical necessity, admission status decisions, and documentation requirements for insurance authorization and billing.
- Serve as an advocate for patients and families.
- Evaluate the need for Social Service intervention on each patient to determine and provide the levels of assessment and services which are appropriate.
- Work with the patient and/or family to complete intra-agency forms, such as swing bed admissions forms, notice of coverage, and Release of Information.
- Coordinate inter-agency collaboration directed towards patient’s medical care, i.e., Home Health referral, nursing home placement and referrals to other outside agencies and services.
- Develop and/or update applicable departmental policy and procedures annually or as needed.
Education and experience required:
- Bachelors of Science Nursing Degree
- Current licensure as Registered Nurse (RN) in Minnesota
- Three years experience in hospital setting; OR combined years of relevant education and experience to perform the essential duties of this position.
- Maintain active BLS Certification
