Weld County Department of Human Services (DHS) is expanding our Long-Term Care Services and Supports Case Management Program and looking to hire individuals who want to make a positive impact in the lives of others in the community. Responsible for the direct supervision and development of Case Management Agency staff and ensures that performance standards are met as identified statutes, regulations, and contract guidelines. The position will normally start at $82,430.04 annuallyThis position is exempt from the minimum wage and overtime requirements of the Fair Labor Standards Act.As an applicant for this position, you should be aware of Senate Bill 19-085, the Equal Pay for Equal Work Act. To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Supervision (25%):Directly supervises the Case Management Agency program staff. Provides leadership, guidance and support to staff ensuring a high level of performance and compliance with County, State and Federal rules and regulations. Addresses employee concerns and resolve problems in collaboration with Department Personnel and County Human Resources. Manages hiring process including candidate review, coordinating interview teams, facilitating interviews, and hiring recommendations.Assists in developing educational and professional development opportunities for staff. Encourages integrity and excellence in Case Management, models and expects the Case Management Agency team to be open and flexible to new and different approaches and responsibilities. Provides coaching and assistance as needed to case managers to attain and hone required skills and knowledge. Case Management Oversight (25%):Maintains quality assurance levels for all CMA Programs through case audits, client satisfaction surveys, and case consultations to ensure all program and contract requirements are met. Reviews documents produced by Case Management Agency staff for accuracy of information and content.Shadows Case Management staff completing the Level of Care Screen and other Case Management duties in accordance with state requirements and documents accordingly. Promotes, supports, and utilizes person centered principles and practices. Maintains an accurate client reporting system and submits timely reports as required by contract and regulations and internal monitoring requirements.Remains informed of and complies with latest rules, regulations, and legislative issues that may impact the Long-Term Care System.Participates in the budgeting process, demonstrating a clear understanding of the funding mechanisms of the Case Management Agency. Workflow Management (20%):Manages all the components of assigned waiver programs and any assigned state general fund programs. Assigns and schedules work within the unit using assigned employee's performance and level of skill to assure coverage and efficiency in caseload handling while accomplishing program goals accurately and timely. Monitors and evaluates workflow using knowledge of business functions, program requirements and economic conditions to establish unit goals and timetables. Uses tracking data to identify trends in customer volume and the need for making changes in work assignments to adjust accordingly. Conducts individual and group conferences to set and monitor deadlines. Develops systematic methods to monitor work to ensure compliance. Ensure completion and submission of all required paperwork related to Medicaid appeals. Represent (or designate) the CMA and HCPF at hearings. Outreach (15%):Serves as an advocate for clients, families, and/or providers during conflict situations and assists with problem solving the best solutions to meet clients' long-term care needs as well as for meeting other needs that may arise. Establishes and maintains positive working relationships with community partners and providers, including Program Approved Service Agencies (PASA). Attends all appropriate local and state meetings, committees, workshops, and conferences. Communicates effectively and works in conjunction with local, state, and federal entities to ensure client access to services. Professionalism and Teamwork (10%):Balances team and individual responsibilities; puts success of team above own interests.Exhibits objectivity and openness to others' views; gives and welcomes feedback.Treats others with respect and consideration regardless of their status or position Contributes to building a positive work culture.Accepts responsibility for own actions and follows through on commitments. Other Duties as assigned (5%):Follow policies and procedures, the Weld County Code of Conduct and adhere to strict confidentiality guidelines and appropriate release of information outlined in both.Other duties may be assigned. Any one position may not include all duties listed, nor do the listed examples include all duties which may be found in positions of this class.REQUIRED QUALIFICATIONS A bachelor's degree; OR Five (5) years of experience in the field of Long-Term Services and Supports (LTSS), which includes Developmental Disabilities, OR A combination of education and relevant experience appropriate to the requirements of the position. Relevant experience is defined as: (a) Experience in one of the following areas: long-term care services and supports, gerontology, physical rehabilitation, disability services, children with special health care needs, behavioral science, special education, public health or non-profit administration, or health/medical services, including working directly with persons with physical, intellectual, or developmental disabilities, mental illness, or other vulnerable populations as appropriate to the position being filled; AND (b) Completed coursework and/or experience related to the type of administrative duties performed by case managers may qualify for up to two (2) years of required relevant experience.One (1) year of competency in pertinent Case Management knowledge and skills. This is defined as Long-Term Services and Supports Case Management or the performance of similar duties and responsibilities in a directly related human services or public assistance field.Experience working with aging and disability populations.Experience working collaboratively with community partners.Candidate must pass WCDHS background checks prior to employment start date. Background checks include a Colorado Adult Protective Services check and will continue to be monitored throughout employment. Candidate must possess a driver's license and adequate Liability Insurance by the employment start date and maintain a valid license for the duration of employment.Candidate must possess the ability to learn and apply new computer skills on a regular basis. Candidate must possess knowledge of computer programs such as Microsoft Windows and Word, Excel, and Outlook. Candidate must possess the ability to read and comprehend instructions, correspondence and department policies and procedures. Candidate must possess the ability to write correspondence.Candidate must possess the ability to effectively communicate with diverse populations in a variety of settings, includes public speaking.Candidate must possess the ability to apply common sense to problem solve in a work environment.Candidate must possess the ability to adapt to fast paced, high volume and ever-changing work environment and be able to handle high stress situations.Candidate must possess the ability to make decisions, exhibit sound and accurate judgement, and include appropriate people in decision making process. Candidate must possess a broad working knowledge of human service resources with particular emphasis on the aging network and services for the individuals with disabilities. Candidate must possess the ability to adhere to strict confidentiality guidelines and appropriate release of information.PREFERRED QUALIFICATIONS Experience with Long Term Services and Supports waiver programs and/or State General Funds Programs.Experience supervising and managing employees that includes directing work activities and evaluating employee performance.PHYSICAL REQUIREMENTSThe physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Reliable, predictable attendance within department business hours of 8 a.m. to 5 p.m. Monday through Friday. Work may involve time away from a standard office environment. Work may occur in various community settings that are potentially hazardous and may require additional precautions, such as personal protective equipment and/or requesting assistance from emergency medical personnel or law enforcement. After hours and on call work situations may occur on occasion to provide support to families that need services outside of normal business hours.This position is in-person with a possibility for limited telework opportunities when needed.Employment Type: Regular, Full-time
