Position Summary:


The Behavioral Health Case Manager, as part of the Philadelphia Behavioral Health System, works as member of a multi-disciplinary team that provide short-term outreach services to consumers (adults and children) and families who present with high acuity Behavioral Health needs. The Case Manager will work in collaboration with other service providers associated with the consumer and/or family (making linkages to new service providers when appropriate) to ensure coordination among physical health services, Behavioral Health services and other social services. The primary job function will be to provide case coordination and continuity of care planning by directly assisting the consumer and/or family to navigate the various systems.

Duties and Responsibilities:
  • Assess present strengths and level of functioning to develop client-centered and/or family-focused personal plans.” Consultation with social services and hospital sources will serve to enhance the comprehensiveness of the information.
  • Participate in inter-agency planning meetings.
  • Provide outreach and engagement to consumers and families who have been identified as having high acuity of need.
  • Work with other members of the COC Unit to ensure that coordination of Behavioral Health care services goes uninterrupted from the assessment process through the treatment/services options with re-assessment when appropriate.
  • Assist the identified consumer and/or family members to “navigate” the multiple systems involved in the provision of services that are necessary for stabilization and successful re-integration into the community.
  • Provide coordination/linkage, monitoring and advocacy for the consumer and/or family with social service and other community resources. Provide linkage to educational and vocational opportunities, to Behavioral Health services and to physical health care in age and culturally-appropriate programs.
  • Provide assistance to the consumer and family as linkages are made which enhance the ability to establish, improve and maintain community living.
  • Focus on the assessment of needs, skill development related to these needs, problem resolution, establishment and building of support systems, acquisition of entitlements, etc. to assist in linking to longer term services.
  • Maintain current information on available resources and adjunctive components of the DSS as well as on the necessary processes and procedures for accessing same.
  • Advocate on behalf of the consumer and/or family to have access to the services and supports necessary for success.
  • Demonstrate willingness to accept other tasks, as assigned by designated supervisor and to maintain a flexible work schedule.
  • Bilingual (English/Spanish) is strongly recommended; but not a requirement.
Education and Experience:
  • A minimum of a Bachelor’s Degree in a recognized Behavioral Health field with a minimum of two (2) years’ experience in the Mental Health and/or Drug and Alcohol field in either or both child or adult component of the social services system; experience in a combination of at least two (2) of these areas and age groups is preferred.
  • Active participation as a member of a treatment team, preferably community based treatment, minimum two (2) years’ experience in this capacity. Additional consideration will be given to those applicants who are well versed in the children’s component of the Social Services system within Philadelphia.
  • Have all appropriate levels of “security’ clearance. Valid motor vehicle operator’s license with proof of insurability, prior to employment.
