POSITION OVERVIEW
Under the supervision of the Project Coordinator, the Case Aide is responsible for case support to the Case Manager and for all data entry.
Starting Salary: $28/hr
QUALIFICATIONS
- A Bachelor's degree in a Behavioral Science preferred.
- At least 2 years working in Case Management or data entry.
- At least 2 years working with at risk families.
SKILLS AND KNOWLEDGE REQUIRED
- Good verbal and written communication skills.
- Requires a valid CA Driver's License and a good driving record.
- Must possess or be able to obtain current CPR and First Aid certificates.
- Must have computer skills in word processing and data entry programs.
- Must meet agency conditions for employment including clearance for Child Abuse.
RESPONSIBILITIES
- Provides case aide support to the Case Manager in doing outreach to potential clients and in making referrals to other agencies for services outlined in client treatment plans.
- Provides support and transportation for clients to outside providers appointments.
- Maintains client files to meet all compliance requirements.
- Completes all data entry as required in an accurate and timely manner to meet grant requirements.
- Maintains resource files, keeps forms and information packets current.
- Provides support for clients in enrolling in vocational, GED, or college classes.
- Maintains client confidentiality in all circumstances as required by professional ethics and legal requirements.
- Maintains any calendar, attendance, and sign-in records as required.
- Communicates effectively with coworkers, clients and personnel associated with outside agencies.
- Performs effectively and professionally under pressure.
- Attends all team meetings and in-service training courses as assigned.
- Supports and implements the Epiphany Center mission and values.
- Other duties as assigned.
