As a Caregiver with SYNERGY HomeCare, you will:

• Oversee and area of clients, ensure their needs are being met
• Develop and maintain communication with the Case Manager
• Assist client with light housekeeping, meal preparation, and medication reminders
• Identify any Plan of Care updates
• Establish communication and a professional relationship with clients, family members, and co-workers
• Provide reliable care by being punctual and consistently covering shifts
• Organize your schedule and ensure clients are being serviced as per requirements
• Participate in monthly meetings with Case Managers

Lead Caregiver Requirements

• CPR and First Aid
• TB reading negative
• Level one Fingerprint Clearance Card
• Direct Care Worker Certification- We offer for FREE!
