Position Summary
Summary:
The Care Manager is accountable for the management and coordination of the patient’s plan of care beginning with the admission, active phase of care, and addressing of discharged needs and concerns to ensure the patient receives appropriate, cost effective, quality care during their in-patient stay.

Education Required
- Must complete in-house orientation program
- Graduate of an accredited Registered Nurse diploma or college program.
Experience And Skills
- Excellent interpersonal skills
- Speaks, reads and writes English
- 5 or more year’s progressive hospital experience.
- 2 or more year’s hospital case management, utilization reviews or discharge planning experience.
- Good oral and written communications skills.
- Good interpersonal skills.
License/Certifications
- Licensed and currently registered or otherwise duly authorized to practice as a professional nurse in New York State.
- PRI certifications.
- Case Management certification strongly encourage.
- Bachelors Degree in Nursing preferred.
