AngleZ Behavioral Health Services is currently seeking a Care Coordinator who specializes in co-occurring disorders to join our Behavioral Health Home team! This position would work closely with our Opioid Health Home Team to provide Care Coordinaton and collaboration. The successful candidate must have significant experience with individuals working towards recovery and stability.
The Co-occurring Health Home Coordinator will manage the following:
- Positive and Pro-social Communication between the member and their support teams.
- Coordinate with the team during the development of the Plan of Care.
- Monitor and coordinate the plan of care as it relates to the goals associated with community resources and stability and communicate progress.
- Identify and link member with community resources.
- Facilitate referrals and respond to social service needs.
- Collaborate with other team members including, MAT prescribers, doctors, nurses, other Care Coordinators, and clinicians.
- Monitor and assist them in problem solving potential issues related to the health care system, financial or social barriers.
- Accurate documentation in an electronic health record
- Positive relationships with Sober Living Homes, Recovery Housing, Residential facilities, Justice systems, and Low Barrier Shelters.
MHRT/C Required, BA degree preferred but experience is an acceptable substitute.
Job Type: Full-time
Pay: $21.00 - $24.00 per hour
