SalaryThe Care Coordinator will be in charge of recruiting new residents, coordinate their intakes, and promote a good relationship between residents -staff, the OLHC with guardians and family. The following duties are and not limited to:
- Provide intake/assessment before the client's placement and ongoing assessment as required
- Developing and coordinating health care programs for each resident
- Communicating between staff, residents s and family
- Handling Residents case management and education
- Recruiting new residents
- Developing residents goals and monitoring progress
- Maintaining resident confidentiality and quality care
QUALIFICATIONS:
MHRT/C or MHRT/C eligible. Bachelor's/Associate degree in Social work, Behavioral Science or a related field preferred.

This position pays $25.00 to $30.00 per hour depending on experience.

Expected hours: No more than 30 per week
