Job description

I. POSITION PURPOSE:


Under the supervision of the MAT Supervisor, the Care Coordinator works in an interdisciplinary manner with CHC clinical care teams and community partners. Primary responsibilities include developing longitudinal relationships with complex patients, promoting preventative health priorities, decreasing and eliminating barriers to patients’ access to health care, providing coordination of care among health care providers and facilitating transitions of care.

BASIC QUALIFICATIONS:


A) Education & Experience:


  • Associate degree in related field such as psychology, public health, strongly preferred.
  • Prior experience in a health care, mental health or community services setting providing social services or case management services strongly preferred.


B
) Knowledge, Skills & Abilities:


  • Proficiency in using Office Suite (Word, Excel and Outlook)
  • Experience using Electronic Medical Records
  • Excellent verbal and written communication skills and ability to maintain professional demeanor.
  • Basic understanding of medical terminology.
  • Demonstrate kindness, compassion and empathy for all individuals.
  • Ability to work as part of a multidisciplinary care team.
  • Ability to operate standard office/IT equipment such as a fax, scanner and printer.

III.
ESSENTIAL DUTIES (see Section VIII):


  • At CHCB the primary accountability of the Care Coordinator role will be to ensure safe,

timely, efficient and effective transitions of care for patients – both within and outside of the primary care practice.

  • Participate as a member of the multi-disciplinary care team to facilitate the care of high risk, vulnerable, or complex patients.
  • Actively engage patients and family members in the creation of patient-centered goals
  • Identify and collaborate with community agencies and community-based care managers to ensure coordination of care.
  • Create and nurture effective linkages and relationships between patients and the health care system.
  • Advocate for patients to receive appropriate services.
  • Educate and support patients to access health care and community resources.
  • Document assessments, care plans and interventions in the electronic medical record.
  • Communicate in a timely manner with primary care providers and other care team members.
  • Promotes diversity and respects coworkers, patients and their families regardless of race, color, sex, sexual orientation, gender identity or expression, ancestry, place of birth, HIV status, national origin, religion, marital status, age, language, socioeconomic status, physical or mental disability, protected veteran status or obligation for service in the armed forces.
  • Provide support to population health priorities and quality goals of the organization such as promoting preventative and chronic health recommendations (e.g., breast cancer screening, A1C screening).
  • Works collaboratively with all staff, providers and leadership
  • Attend staff meetings.

  • Provides a positive patient-centered experience for every patient.
  • Considers safety of patients and works to help provide a safe environment.
  • Maintains a current up-to-date knowledge of all policies and procedures.

  • Supports programmatic goals and priorities including tracking patients receiving care management services.
  • Participated in quality improvement projects.

V.
PHYSICAL DEMANDS AND WORKING CONDITIONS (see section VIII):


  • Ability to lift up to 25 lbs.
  • Work is performed in an indoor, controlled environment workspace.
  • Exposure to infectious disease by way of patient contact.

Relationships
: All CHCB employees must maintain professional and effective working relationships with patients, staff and the public.


Customer
Service:All CHCB employees must provide high quality customer service to both internal customers (fellow employees), and external customers (patients and business associates) on a consistent basis.


Confidentiality
: All CHCB employees must protect the confidentiality of all patients, patient care, staff/personnel information, business and proprietary information, and financial information. Employees must read and acknowledge receipt of CHCB’s Confidentiality Policy.


Compliance
: Employees must complete required trainings and adhere to the Code of Conduct, and policies in the Employee Handbook. Employees must read and acknowledge receipt of CHCB’s Code of Conduct and Employee Handbook.
