JOB SUMMARY: The office-based Nurse Care Coordinator works collaboratively with the nursing and program teams to provide care management support across agency programs and to ensure proper oversight of medical documentation and information.  The Nurse Care Coordinator plays a key role in ensuring the agency meets its medical care quality standards. 

 

ESSENTIAL FUNCTIONS: (Majority of duties, but not meant to be all-inclusive nor prevent other duties from being assigned as necessary.) 

  • Collaborate with agency teams as well as community-based medical providers to support the delivery and proper documentation of medical care.  
  • Manage and/or support communication with medical provider offices and pharmacies as assigned.  
  • Meticulously review specified medical documents for quality and maintain a shared recording system with the nursing team to support effective oversight.  
  • Accurately, clearly, and promptly transcribe all medication orders into the Electronic Medical Record per agency guidelines.  
  • Provide support to service coordinators with creating Medication Administration Records.  
  • Reconcile and document all client-related medical information promptly, comprehensively, and clearly as indicated.  
  • Receive and process all incoming medical documentation (annual physicals, specialty exams, lab reports, medication administration records, etc.)  
  • Provide support to teams in securing, reconciling, and/or disposing of medications as needed.  
  • Support in maintaining the supply and organization of work tools, supplies, and spaces.  
  • Other medical-related duties as assigned  
  • Participate in staff meetings, team meetings, and agency events as indicated  
  • Support with nurse scheduling as needed  
  • Collect and report data pertaining to Quality Assurance goals.  

 

 

KNOWLEDGE AND EXPERIENCE: (Minimum education, experience, technical and communication skill levels, and licenses/certificates normally required to perform the duties of this position.) 

 

  1. Licensed Practical Nurse Certificate 
  1. Vermont licensure, in good standing  
  1. Nursing knowledge and commitment to understanding the particular medical needs of those we serve  
  1. Exceptional attention to detail 
  1. Excellent oral and written communication skills 
  1. Ability to organize one’s work and workflows with efficiency and effectiveness 
  1. Ability to maintain and model a calm, positive demeanor in stressful situations 
  1. Ability to collaborate effectively and adapt work style to meet teams’ needs 
  1. Ability to maintain confidentiality. 
  1. Proficiency with Microsoft 365 and ability to acquire fluency in other needed applications. 
  1. Ability to multitask, manage interruptions, and stay focused on priority work

WORKING CONDITIONS: (Typical working conditions associated with this type of work and environmental hazards, if any, that may be encountered in performing the duties of this position.) 

 

Internal – Work is normally performed in a climate-controlled office environment, where exposure to conditions of extreme heat/cold is very limited. Noise level includes sounds of normal office equipment (computers, telephones, etc.).   

External – Travel to participants’ homes, hospitals, provider offices, and training sites is occasionally required.  Rarely, exposure to bodily fluids and needle sticks may occur.  Rarely, exposure to dysregulated individuals who may demonstrate verbal or physical aggression may occur. 

 

PHYSICAL DEMANDS: (The physical effort generally associated with this position.) 

 

Employee is frequently required to walk, talk and/or hear. Duties are normally performed from a seated or standing position. Employee may be required to sit for long periods of time.  There is potential for eyestrain from reading computer screens. Work may include occasional pushing, pulling, or carrying objects weighing approximately 50 pounds such as files, documents, equipment, and soft goods. Work normally requires the ability to communicate
