This is position is responsible for providing management of CAP case management services to medically fragile children and adults who are at risk of institutionalization and whose health, safety, and well-being can be maintained in his/her primary residence. The Program Manager will coordinate services with RN and Social Workers to ensure quality care.
The Community Alternative Program for Children is a Community Based Services Waiver designed to provide an alternative placement in a nursing facility for beneficiaries who prefer to remain in his or her primary private residences and would be at risk of institutionalization without these services. We work with individuals to establish the person-centered plan and to coordinate care and services including personal care, waiver supplies, home modifications and educational support.
Pay: $58,000
Requirements:

• Master's degree preferred in a related human service field plus five years of experience in program development. Considerable knowledge of/experience with: budget management, policy development, laws, statutes and standards affecting service delivery. Individuals with a bachelor degree in a human service field can be considered with a combination of related experience
• Required Licensure/Certifications
• Valid NC driver's license and a clean driving record (this is a condition of employment) Personal auto liability insurance is mandatory if transporting clients in private/personal vehicle(s). Must meet all other conditions of employment related to criminal record screening, drug screening, reference checking, etc

Responsibilities:

• Reassign caseloads to case managers as needed
• Assigning CNRs to case managers monthly
• Track caseload capacity and caseload balance
• Tracking late CNRs and assign CNRs monthly

• Tracking unreleased CNRs
• Tracking eligibility: ensure initials in eCAP have correct effective dates and to coordinate with CM & DSS
• Tracking of incident and complaints to ensure follow through and closure
• Complaint management, coordination of grievance and complaints
• Tracking CapC kids aging out to ensure transition process
• Complete necessary reporting information to include but not be limited to caseload assignment, billing completed verses what is outstanding, wait list information, chart reviews completed, and corrections completed.

• Creation of monitoring tools, forms and templates for case manager utilization
• Assist with the development of a Plan of Correction for programs that have identified deficiencies through the Quality Assurance program or external surveys, and routinely monitor progress toward implementation of the Plan of Correction.
• Staff training and support operationally with Eclipse, ECAP, CCNC, NCTracks, FME portals, e-files, electronic signatures, printing, remote faxing, and upload folder

• Request mailings to go out on a weekly basis for Welcome letter, Withdrawal letter, Termination letter
• Review and approving timesheets weekly and monthly, PTOs
• Conducts an Annual Program Evaluation and develops annual quality improvement goals and outcome measures for programs.
• Interviewing, hiring, and training Case Managers (SW's and RN's)
• Responsible for policies and procedures
• Coordinate with the counties Child Protective Services and Adult Protective Services Department

Regularly required to lift 30 lbs. Must be able to lift a minimum of 70 lbs. Must be able to pull minimum of 50lbs. Must be able to squat, kneel, crawl, crouch, climb, and stoop. Must be able to regularly use hands to finger, handle, or feel objects, tools, or controls. Required to regularly stand and walk. Must be able to demonstrate proficiency in CPR from the floor level requiring to work on hands, knees, bending, standing and lifting. Vision requirements include close vision, distance vision, and peripheral vision. Must be able to talk and hear
