The Case Work Supervisor is responsible for providing and overseeing casework services for runaway and homeless youth at the Brooklyn Youth Center. The Case Work Supervisor will carry out all of the following duties and actively collaborate on ideas to improve client outcomes. The Case Work Supervisor works in collaboration with the Brooklyn Youth Center Project Coordinator.
SPECIFIC RESPONSIBILITIES:
Responsible for maintaining a revolving caseload and completing all required progress notes and paperwork in full compliance with City (DYCD) regulations and standards.
Supervises Case Manager/Housing Navigators in all aspects of their jobs: individual/family meetings; educational/vocational and employment services, entitlement, housing (emergency referrals, housing applications/search/placement), legal and entitlement assistance, mental health/health services, etc.
Promotes and ensures superior case management services including developing appropriate client resources, reviewing all client documentation, ambitious family involvement, connection to health/mental health care, and meaningful educational and vocational programs.
Oversee Youth Specialists regarding their specific responsibilities supporting case management services.
Complete/review intake assessments for youth in need of shelter or other services
Complete Service Plan reviews in order to assist clients meet their short and long-term goals. Monitors and reviews Services Plans for each client completed by BYC staff.
Provide crisis prevention and safety planning services
Assist with Brooklyn Youth Center community and program related forums when necessary
Report any suspected abuse or neglect to the Statewide Central Register
Review and assists in completion of reports and data entry as required by City/State and SCO such as monthly reports, annual reports, Key Performance Indicators, DYCD Connect.
Monitors and maintains training documentation and performance evaluations for Case Manager and Housing Navigators.
Act as on-call administrator in the Project Coordinator’s absence.
Creates and ensures the maintenance of the program milieu is safe, caring, and therapeutic.
Conduct periodic case reviews especially related to the preparation of incident reports.
Perform regular policy review and revisions for program specific quality management and improvement activities.
Perform other duties as delegated by supervisor
Qualifications - External
QUALIFICATIONS AND EDUCATION REQUIREMENTS:
Knowledge and understanding of runaway and homeless youth
Flexibility in schedule
Bachelor’s degree with related experience, MSW preferred.
Experience in program supervision preferred.
Bi-lingual is a plus
