The Job
Enhance seniors’ ability to manage their behavioral health and live independently by providing education on behavioral health symptoms and conditions as well as assisting in facilitating the connection to primary care and community services.
You Will
Facilitates Community classes around health enhancing topics
Conducts Outreach and research of resources for those served by the Insights Program.
Advocates for clients in need of higher level of care so that they are connected to the care and services they need.
Facilitates communication between the clients / participates and their providers/service providers to address gaps in behavioral health treatment.
Participates in departmental QA process and assist with program documentation.
Maintains client’s health by advocating for client, helping them understand and use community services, assisting in scheduling appointments, building a support system, and reinforcing use of self-management and organizational tools.
Supports clients / participants well-being by using strong observation and communication skills to coordinate care between physician office, client, caregiver, and community resources.
Assists clients and caregivers in taking an active role in behavioral health management and promotion, by coaching, education, navigation and referrals to appropriate care and community-based resource.
Outreach to educate community agencies, potential clients / participants and provider groups regarding services and how to make referrals.
Exhibits strong interpersonal, critical thinking and analytical skills through positive communication with clients, caregivers, care team and community agencies.
Demonstrates excellent organizational, decision-making and multi-tasking skills as demonstrated by problem solving and successful outcomes.
Completes timely and accurate documentation in multiple computer systems to record assessment, observations and corresponding documentation, including care plans, and progress notes.
Enhances skills and knowledge by participating in educational offerings and team case conferences and training per department guidelines.
Utilizes department desktop procedures, workflows, job aids and training material. Identifies barriers to work processes and brings to the attention of the supervisor/manager.
Maintains the client‘s right to privacy and protects SCAN operations by keeping information confidential.
Adheres to all quality, compliance and regulatory standards to achieve desired outcomes.
Contributes to team effort by accomplishing related results as needed.
Actively supports the achievement of SCAN’s Vision and Goals.
Your Qualifications
Bachelor’s degree in social work or related field. A comparable combination of education/experience and/or training will be considered equivalent to the education listed.
BILINGUAL- Must be bilingual in English/Spanish. (Test will be administered to assess proficiency.)
Moderate travel in Harris County, TX; must live in this territory.
2+ years of experience in community services, preferably a healthcare or behavioral health environment; Medicare/Medicaid experience, preferred.
2+ year experience working with older and disabled individuals and their families, community-based experience, preferred.
Working knowledge of behavioral health terminology, symptoms, and conditions
Understanding of confidential information
Broad understanding of local community resources for seniors and disabled.
Proficient in MS Office.
Ability to communicate with clarity, the vision, value, and tasks to formal and informal audiences.
Able to develop relationships quickly and leverage working relationships to drive performance outcomes.
Ability to work with all levels of staff, within and external to the organization to achieve goals.
Strong interpersonal skills, including excellent written and verbal communication skills.
Strong organizational skills; Ability to multitask and to appropriately maintain confidentiality.
