This role is an Independent Contractor role with the flexibility to determine your own schedule while meeting the timelines and expectations of the company. - Minimum of 2 years' LPN experience. (We are also recruiting for part-time nurses with a License to operate in the State of Georgia and living in Georgia (Preferred Gwinnett and Covington Counties)
- Conducting home assessments under specific guidelines specified by the organization.
- Prescribing care plans for members eligible for a waiver program provided by the state.
- Working closely with other RNs and LPNS
- Assess members health and safety conditions and make determination on level of care required, in their homes.
- Use nursing skills to determine patients/clients needs and make recommendations for home and community-based services.
- Work closely with other departments using a team approach for preferred patient/client outcomes.
- Bi-Lingual a plus: Languages such as Spanish, Korean,. Vietnamese, Russian, etc.
