Job description

The Student Internship Program at Horizons Mental Health Center (HMHC) provide an opportunity for students in a master's level therapy program to gain practical work experience in the area of counseling, behavioral health, and human services. The intern position supports the promotion of individual growth in consumers presenting to HMHC seeking Mental Health support.
The HMHC Assertive Community Treatment (ACT) Team serves individuals diagnosed with severe and persistent mental illness whose needs have not been met by traditional clinic-based services. The ACT team outreaches and enrolls people into its services, provides comprehensive Mental Health and social service support, helps to obtain and maintain housing, works with individuals to return to a full and satisfying life in the community. The ACT Mental Health Therapist provides services in the community and as a part of our ACT Team. This includes delivering services such as wellness self-management counseling, integrated dual disorder treatment, and assisting to improve daily living skills to consumers who have a history of poor engagement in traditional Mental Health services. This position works closely with other treatment teams as needed to provide services at a higher level of intensity or frequency with the goal of the consumer remaining in the community with resources in place.
ESSENTIAL FUNCTIONS:

  • Provides outpatient behavioral health treatment, with the guidance of the university and placement supervisor, to include individual, family and group therapy (caseload determined by hours needed to fulfill internship)
  • Collaborates with individuals being serviced in a compassionate, culturally responsive and trauma informed manner to develop effective treatment plans that reflect the individual's specific needs
  • Completes intakes, diagnostic assessments, and mental status exams on consumers entering services and thereafter every 90 days or as medically necessary
  • Participates in daily team meetings to assign cases to team members and review day's work responsibilities
  • Support consumers to achieve treatment goals, coordinate with collateral contacts, medication education, crisis management, prompt client to complete daily living activities, monitor client one to one for safety or as needed with the support of supervisor and other team members
  • Completes ongoing diagnostic evaluation of consumers and completes required clinical assessments per organization policy and procedure
  • Participates in discharge planning and support transition to the community or to higher level of treatment.
  • Collaborates with team in plans to ensure clients attend Mental Health appointments
  • Contribute to a program culture that is person-centered, outcome-oriented and dedicated to continuous quality improvement
  • Participates in care coordination through collaboration with consumers, family, other treatment team members and community agencies around treatment planning
  • Engages in opportunities to meet with different departments within Horizons to learn about different services offered and tools implemented in a variety of areas to provide high quality Mental Health treatment
  • Articulates all service provisions through timely and quality record documentation.
  • Completes necessary trainings and competency reviews as assigned
  • Participates in individual and/or group supervision on a weekly bases with an assigned placement supervisor and maintains contact in between supervision with other needs
  • Demonstrates an ongoing commitment to the development of therapy skills and evidenced-based practices
  • Other duties as assigned.

MINIMUM KNOWLEDGE AND SKILLS REQUIRED:
  • Knowledge of various theoretical treatments and practice applications.
  • Ability to work with a wide range of ages and life circumstances
  • Professional verbal and written communication skills
  • Ability to learn, understand and utilize various telehealth and documentation software

PREFERRED KNOWLEDGE AND SKILLS REQUIRED:
  • Experience managing treatment plans and working with a treatment team
  • Training in Dialectical Behavioral Therapy, Solution Focused Brief Therapy, Motivational Interviewing, Cognitive Behavioral Therapy and other evidenced based treatments.

MINIMUM EDUCATION AND EXPERIENCE REQUIRED:
  • Must be currently working towards the fulfillment of internship hours under the supervision of an accredited program for a master's degree in a Mental Health clinical field

PREFERRED EDUCATION AND EXPERIENCE:
  • Three to five years of experience working with diagnosing and treating the SPMI/SMI or SED populations.
  • Understand integrating care with other providers.
  • Three to five years of experience working with diverse client population including high-risk populations.

REQUIRED LICENSE/REGISTRATION/CERTIFICATION:
  • All necessary trainings for delivering specialized care, including all applicable trainings for being a Qualified Mental Health Professional (QMHP).
  • Must pass all background checks
  • Valid Driver's License

PREFERRED LICENSE/REGISTRATION/CERTIFICATION:
  • Substance Use Disorder licensure.
