Job description

The Registered Nurse (RN) provides services to consumers being served on the Assertive Community Treatment (ACT) Team. In collaboration with the ACT Team and under the guidance of a psychiatrist, the RN assists with medication management, education, monitoring of medical conditions and liaising with medical providers. The RN is a part of the multi-disciplinary team to provide treatment and recovery support services in order to assist mental health consumers in avoiding more restrictive levels of treatment. Some on-call/after-hours work may be required under the guidance of the on-call Psychiatrist.
Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.
ESSENTIAL FUNCTIONS:

  • Under the guidance of a psychiatrist, coordination of health care provided to consumers in the community
  • Participates in routine team meetings to discuss consumer needs and action plans
  • Participates in medication administration, evaluation of medication, management of medication orders, delivery of medication and monitoring in the office, community, or consumer's home
  • Acts as the initial responder for phone calls from consumers on the ACT Team, their families, and care agencies regarding patient care issues, medication refills and problems such as medication side effects.
  • Uses broad based knowledge and/or medical provider consultation to provide counsel and recommendations for clients, family members and other Horizons personnel providing services to consumers.
  • Assists with the review/monitoring of lab work ordered by the medical provider, alert medical provider of abnormal results and record findings in the medical record
  • Assists in identifying needed resources and ensuring participants access primary care and receive annual physical, dental, and other relevant examinations
  • Provides rehabilitation and support services under clinical supervision
  • Contributes to the provision of quality nursing care through techniques that support and achieve positive outcomes
  • Utilizes motivational interviewing techniques and trauma informed approaches when delivering services
  • Administers injections under the direction of a psychiatrist
  • Monitors vital signs and side effects of medications
  • Conducts psycho-educational programs for consumers of the ACT Team, their families and other supports and possibly the community
  • Advocates for consumers to secure medical care and other community supports through care coordination efforts
  • Assist with ordering and maintenance of medical supplies and equipment
  • Requires some on call on evenings and weekends as scheduled
  • Frequent driving through use of company vehicle
  • Assesses level of risk and immediacy of the service needed.
  • Articulates all service provisions through timely and quality record documentation.
  • Abides by the Health Insurance Portability and Accountability Act (HIPAA) privacy and security regulations regarding all aspects of Protected Health Information (PHI).
  • Participates in tracking relevant data for the ACT Team
  • Completes documentation in a timely manner
  • Completes related activities as assigned by the director
  • Other duties as assigned.

MINIMUM KNOWLEDGE AND SKILLS REQUIRED:
  • Working knowledge of criteria used for diagnosis of mental disorders.
  • Working knowledge of medical disease processes that may be impacted by mental disorders and their treatments
  • Maintains a current broad-based knowledge of the pathophysiology of medical disease processes, psychiatric illness, and psychopharmacology

PREFERRED KNOWLEDGE AND SKILLS REQUIRED:
  • One to three years' experience working in a behavioral health setting.

MINIMUM EDUCATION AND EXPERIENCE REQUIRED:
  • Graduate of an accredited Registered Nurse program

PREFERRED EDUCATION AND EXPERIENCE:
  • One year or more experience in inpatient or outpatient behavioral health nursing

REQUIRED LICENSE/REGISTRATION/CERTIFICATION:
  • Current licensure as a Registered Nurse in the State of Kansas.
  • Healthcare Provider BLS - if BLS is not American Heart Association, AHA BLS is required within three months of hire
  • Valid driver's license, insurance, and a dependable vehicle

  • American Heart Association BLS
