What You'll Do:
Play an instrumental role in our program, aiming to deliver medical and social support to our patients where they need it most, in their homes.
Creating Care plans for chronically ill frail elderly seniors.
Educate patients on the importance of consistent health monitoring, medication compliance, and chronic condition management.
Work collaboratively, participating in weekly interdisciplinary team meetings to craft tailored care plans, bridging potential gaps in care.
Remain updated with current clinical standards of care, showcasing your ability to operate independently and prioritize patient needs.
