Monday:8a-4:30p, Tuesday: 8a-4:30p, Wednesday: 8a-4:30p, Friday:8a-4:30p
Major Responsibilities:
- Facilitate/Co-facilitate therapeutic and educational groups for all Partial Hospital Program (PHP) and Intensive Treatment Program (IOP) patients.
- 1)Brief individual counseling session per week.
- 2)Research and update group topics and communicate group ideas to coworkers based on milieu needs.
- 3)Organize and provide education materials for groups.
- 4)Provide supervision to associates/students currently seeking LCPC/LCSW licensure.
- Document individual group participation based on the individualized treatment plan, assign goals, implement the plan, monitor the progress and document all of the above in accordance with Illinois Mental Health Code and Good Samaritan Hospital Policies and Procedures.
- 1)Prepare, document and maintain accurate patient records including assessment, progress and discharge notes as well as attendance and services rendered.
- 2)Assist with short term disability paperwork for patients currently enrolled in the program.
- Participate in multidisciplinary staffing in order to provide quality patient care.
- Complete social work assessments on all patients admitted to the PHP/IOP program within 48 hours (excluding holidays and weekends)
- 1)Act as a liaison between hospital and community therapist, counseling agencies, public agencies (i.e.: nursing homes, DCFS, Probation, OMH), and schools, to maintain continuity of care upon discharge.
- Plan and implement family session according to the patients goals outlined in the multidisciplinary treatment plan.
- 1)Contact appropriate family members.
- Establish and maintain effective communication, and working relationships with the medical staff, unit manager and the multidisciplinary staff. Participate in unit and program meetings.
- 1)Participate in unit and program meetings.
- Assists in promoting units/services.
- 1)Participates in hospital marketing (i.e. community education seminars)
Licensure:
- Licensed Clinical Social Worker; or Licensed Clinical Professional Counselor
Education/Experience Required:
- Masters Degree in Counseling or Social Work. LCPC or LCSW with 2 years experience in a psychiatric setting
Knowledge, Skills & Abilities Required:
- Good assessment skills Good organizational skills Good communication skills Good computer skills i.e. Microsoft Word
- Crisis Prevention Intervention CPR certification Current licensure in the state of Illinois as a Licensed Clinical Professional Counselor or Licensed Clinical Social Worker
Physical Requirements and Working Conditions:
- Additional training/experience in family therapy. Resiliency and flexibility to deal with a changing environment. Demonstrated problem solving ability. Ability to work under stressful conditions and in difficult situations. May be exposed to hazardous materials and life threatening diseases. knowledge of universal precautions is a must. Works within and supports a share governance model. Customer focused. Lifting and transporting patient is required. May at time come in contact with violent patients, therefore knowledge of techniques to manage violent patients is a must. May be involved in restraining patients. Ability to flex hours in order to meet patient care needs. Ability to handle crisis situations
- If position has direct patient care or direct patient contact the following lifting requirement supersedes any previous lifting requirement effective 06/01/2015. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case-by-case basis.
